- REQUEST FOR INFORMATION/SOURCES SOUGHT NOTICE Dental Imaging (X-Ray) System; Department of Veterans Affairs, VA Medical Center, (Jefferson Barracks Division) Clinic Building 55 1.0 Description 1.1 The St. Louis VA in support of the Jefferson Barracks Major Projects is seeking information to determine Service-Disabled, Veteran-Owned, Small Business (SDVOSB) or Veteran-Owned Small Business (VOSB) sources with capability to provide Dental Imaging System; brand name or equal to the Kodak Carestream 8100 (G-104344) to include: Dental Imaging Software (DICOM Interface) ( Panoramic X-Ray Unit (Qty. 1) Digital Intraoral Sensors, two (2) difference sizes (Qty. 6) Wall Mounted X-Ray Units (Qty. 4) Wall Intraoral Cameras (Qty. 4) Additional requirements include site visits and installation drawings. The new Dental Clinic space will be comprised of 3 multipurpose treatment rooms and 1 panoramic x-ray room currently under construction at the VA Medical Center, Jefferson Barracks Division, St. Louis MO. The NAICS code for this requirement is 339114, Dental Equipment and Supplies Manufacturing with a size standard of 750 employees. 1.2 THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This RFI is issued solely for information and planning purposes - it does not constitute a Request for Quote (RFQ) or a promise to issue an RFQ in the future. This request for information does not commit the Government to contract for any supply or service whatsoever. Further, the VA is not at this time seeking proposals and will not accept unsolicited proposals. Interested Contractors are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at the interested party's expense. Not responding to this RFI does not preclude participation in any future RFP, if any is issued. If a solicitation is released, it will be synopsized on the Federal Business Opportunities (FedBizOpps) website. It is the responsibility of the potential offerors to monitor these sites for additional information pertaining to this requirement. 2.0 Background A New Clinic Building is being constructed for the St. Louis VA Jefferson Barracks campus. 3.0 Responses 3.1 Interested parties are requested to respond to this RFI via e-mail only to Karen Munroe, Contracting Officer; email: karen.munroe@va.gov no later than December 1D, 2016. Proprietary information, if any, should be minimized and MUST BE CLEARLY MARKED. Please be advised that all submissions become Government property and will not be returned. 3.2 Interested Offerors shall provide, at a minimum, the following information: Name, mailing address, phone number, fax number, and e-mail of designated point of contact. 3.3. SDVOSB/VOSB, contractor must be registered with VetBiz Vendor Information Pages at www.va.gov/osdbu. 4.0 Summary 5.1 THIS IS A REQUEST FOR INFORMATION (RFI) ONLY to identify SDVOSB/VOSB sources that can provide subject equipment. The information provided in the RFI is subject to change and is not binding on the Government. The VA has not made a commitment to procure any of the items discussed, and release of this RFI should not be construed as such a commitment or as authorization to incur cost for which reimbursement would be required or sought. All submissions become Government property and will not be returned.
