Past Performance Questionnaire Sol # DTMA9117R0005 O'Hara Hall Gym Floor Diagram. Gym Floor Resurfacing & Painting SOW Organization The United States Merchant Martine Academy (USMMA) is an agency of the Maritime Administration (MARAD) managed by the United States Department of Transportation. The USMMA is listed on the National Register of Historic Places (NRHP). Therefore, all work shall be consistent with the Secretary of the Interior's Standards for the Treatment of Historic Properties (36 CFR part 68) www.nps.gov/tps/standards/four-treatments/treatment-guidelines.pdf. All work shall meet or exceed the standards provided in the Secretary of the Interior's Standards for Rehabilitation and guidelines for applying the standards published by the National Park Service, U.S. Department of the Interior 36 CFR 67.7 - Standards for rehabilitation. Scope of Work The USMMA seeks a qualified and experienced contractor to provide labor, material, transportation, tools, equipment and supervision to resurface the O'Hara Hall gymnasium floor bi-annually and paint an additional 4 foot radius restricted area arc around the rim to the backboard under both basketball nets. The flooring area to be resurfaced is approximately 18,000 square feet. The restricted area arc lines are to be two inches wide and yellow in color. See the diagram for locations. Contract Type Firm Fixed Price (FFP) Contract Set-Aside/Size Standard The procurement will be a total small business set-aside under NAICS Code 238330 (Flooring Contractors) with a small business size standard in millions of dollars of $15.0 Period of Performance The anticipated period of performance for the requirement is between December 19, 2016 and December 26, 2016. Site Visit Information In accordancw with FAR-237-1 Site Visit, offerors or quoters are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award. The following factors shall be used to evaluate proposals: Factor 1 - Technical Approach/Experience Factor 2 - Past Performance ( Note: The questionnaire document provided must be completed by the point(s) of contact of relevant contracts/projects. Offerors must provide two (2) similar or relevant past performance projects/contracts ) Factor 3 - Price Technical Approach/Experience and Past performance when combined is slightly more important than price. Other Information Interested offerors must be registered in the System for Award Management (SAM) www.sam.gov in order to be eligible to receive an award from any Government solicitation. Also, offerors must ensure annual Representations and Certifications are current or must be completed on the SAM website. It is the prospective offeror's responsibility to check www.fbo.gov as often as necessary for any posted changes to this combined synopsis/solicitation.
