Description

The Department of Veterans Affairs John D. Dingell VA Medical Center, 4646 John R. Street, Detroit, Michigan, 48201 is hereby soliciting offers to provide Dialysis supplies required for Calendar year 2017 with options for 2018 and 2019 calendar years. The VA anticipates entering into a fixed price Indefinite Quantity, Indefinite Delivery contract with the contractor meeting the Brand Name or Equal requirements and providing the best value to the Government. Contract award will be made on the basis of Brand Name or Equal, best value to the Government with non-price related factors being less important than price related factors. This procurement will be conducted using Part 13.5 of the FAR. NAICS Code is 334510, size standard is 1250 employees. The anticipated issue date is 11/30/2016; the anticipated closing date is 12/8/2016. This procurement is a total SDVOSB set aside. The items below are the required items: Line item 0001: Fresenius Kit, Hemodialysis, Pre-pump Inf, Combiset Bloodlines 03-2622-3 or Equal Estimated Usage is 400 Cases per year. Line Item 0002: Fresenius Filter, Hemodialysis, Dialyzer Hi Flux, Polysulfone, Optiflux 180 DNR, Ster. 0500318E or Equal Estimated Usage is 500 Cases per year. Line Item 0003: Fresenius Filter, Hemodialysis, Dialyzer Hi Flux, Polysulfone, Optiflux 250 NR, Ster. 0500325E or Equal Estimated usage is 400 cases per year. To be considered an Equal IAW FAR part 11.104(b), the item offered shall be FDA approved for use with Fresenius Hemodialysis machines.
