Page 1 of 2 This is a combined Synopsis/Solicitation. The solicitation number is VA250-17-R-0130 Repair Windows and is issued as a Request for Proposal (RFP), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 236220. This requirement is a Service Disabled Veteran Owned Small Business set-aside and only qualified contractors may submit quotes. The VHA NCO 10 - 552 Dayton VA Medical Center requires: Window Repair Building 118. The requested items are specified below: Contractor shall provide all labor, equipment, supplies, and supervision necessary to repair 10 stained glass windows on Building 118 at the Dayton VA Medical Center, 4100 West 3rd Street, Dayton OH 45428. Job consists of removing the windows, strip the windows of lead, clean glass, rebuild and reinstall windows. The work consists of only the lower portion of the windows. Actual windows to be repaired will be identified during a walk through with the contractor. Request for Information Submissions: Interested contractors must submit questions, via e-mail, concerning the solicitation NLT DECEMBER 12, 2016 at 11:00AM (EST) to enable the Contracting Specialist and/or COR sufficient amount of time to respond. Questions not received within a reasonable time prior to close of the solicitation may not be considered. Proposal MUST be good for 120 calendar days. The selected contractor must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. In addition to any VHA terms, the contractor has reviewed and acknowledges any/all attached solicitation documents, item details, delivery schedule, and/or instructions (collectively referred to as requirements), and agrees to all terms and conditions set forth in the solicitation documents.
