TITLE & DESCRIPTION Special Purpose Vehicle Elevator (SPV) The Contractor shall provide all supervision, management, labor, tools, parts, materials, supplies, transportation and equipment necessary to perform preventive maintenance, routine and emergency repairs of the SPV at the Schofield Barracks deep well water plant on the island of Oahu, Hawaii. 2. PERIOD OF PERFORMANCE Base: 25 Jan 2017 to 24 Jan 2018 Option Period 1: 25 Jan 2018 to 24 Jan 2019 Option Period 2: 25 Jan 2019 to 24 Jan 2020 Option Period 3: 25 Jan 2020 to 24 Jan 2021 Option Period 4: 25 Jan 2021 to 24 Jan 2022 3. REQUEST FOR QUOTE INFOMRATION The quote is due in this office by: 0900 HST on 03 January 2017 Documents must be submitted to: a. E-MAIL: vanessa.r.westmoreland2.civ@mail.mil OR b. MAIL OR SPECIAL CARRIER: 413th Contracting Support Brigade RCO-HAWAII 742 Santos Dummon AVE. Schofield Barracks, HI 96857 ATTN: VANESSA WESTMORELAND ATTN OFFERORS In responding to this request for quote (RFQ), you must provide pricing information for each CLIN on the solicitation, ***CLINS 0005, 1005, 2005, 3005, 4005, 0007, 1007, 2007, 3007, and 4007 require a labor rate per hour, ensure you meet the evaluation criteria as out lined in the solicitation and respond within the appropriate time frame. **If your response does not contain all of the required information called out in this notice, your quote will be determined non-responsive and will not be considered for award.** 4. QUESTIONS Interested offerors must submit any questions concerning the solicitation at the earliest time possible to reasonably expect a response from the Government. Questions must be EMAILED to vanessa.r.westmoreland2.civ@mail.mil (PHONE CALLS WILL NOT BE ACCEPTED) Questions must be submitted before (28 December 2016) (0900 HST).
