THIS IS NOT A SOLICITATION ANNOUNCEMENT. THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. THE PURPOSE OF THIS ANNOUNCEMENT IS FOR MARKET RESEARCH AND TO GAIN KNOWLEDGE OF POTENTIAL QUALIFIED SOURCES AND THEIR SIZE CLASSIFICATION. THIS NOTICE IS PARTICULARLY SEEKING ANY SDVOSB OR VOSB THAT HAS AN AFFILIATION AND PRIVILEGES WITH OHIO STATE UNIVERSITY MEDICAL SCHOOL PROGRAM AND THAT CAN PERFORM THIS WORK AND CAN SUPERVISE ITS RESIDENTS IN THE AREA OF OPTHALMOLOGY. RESPONSES TO THIS ANNOUNCEMENT WILL BE USED BY THE GOVERNMENT TO MAKE APPROPRIATE ACQUISITION DECISIONS. A SUBSEQUENT SOLICITATION ANNOUNCEMENT MAY FOLLOW. RESPONSES TO THIS ANNOUNCEMENT WILL NOT BE CONSIDERED AS AN ADEQUATE RESPONSE TO ANY FUTURE ANNOUNCEMENT. The Dayton VA (DVMC) in Dayton, Ohio has a requirement for Ophthalmology Services to provide service for the Center s patients within the Greater Dayton, Ohio area. The Medical Center will be seeking to solicit a Firm Fixed Price contract in the near future. Interested parties should provide the following: Company Name, Company Address, Point of Contact, (POC) phone number/email address, DUNS number and capability statement for the services as stated above. Responses to this notice should include three customer references to which you have provided similar services to, copies of applicable licensing, and the size of your business pursuant to the following questions: (1) Is your business large or small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified hub zone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business. Those wishing to identify their capability to provide the afore-mentioned service should respond in writing to this announcement via email to Glen.Johnson@va.gov with the Subject matter in the email Ophthalmology Services Capability or mailing address to Glen Johnson, Contracting Officer, Network Contracting Office (NCO) 10, 6150 Oak Tree Blvd., Suite 300, Independence, Ohio 44131, no later than January 23, 2017 at 12:00 noon Eastern Standard Time. No telephone inquiries will be accepted. All vendors must be registered in System for Award Management (SAM) at www.sam.gov. THIS IS A REQUEST FOR INFORMATION ONLY, OR SOURCES SOUGHT, SHALL NOT BE CONSTRUED AS A REQUEST FOR PROPOSALS OR OFFERS. INFORMATION IS TO BE PROVIDED ON A VOLUNTARY BASIS.
