Description
The Department of Veterans Affairs, Network Contracting Office 10, 24 Frank Lloyd Wright Drive, Lobby M, Suite 2200, Ann Arbor, MI 48105, intends to solicit offers for Courier Services for and between the Veterans Resource and Referral Center and the John D. Dingell VA Medical Center. Contract will be for the period from Date of Award through one year with four (4) option years in strict accordance with terms, conditions and provisions of the contract solicitation, NAICS Code 492110. Contractors that can provide this requirement and meets the requirements below are requested to send their 1. Company Name, 2. DUNS number, 3. Business size under NAICS 492110, and 4. If the item(s) are available on a FSS contract or not and the FSS contract number if it is available on contract, 5. Notification if they are a VETBIZ registered VOSB/SDVOSB or not. This is not a solicitation. This is a Request for Information, Sources Sought. This Sources Sought will close on 01/12/2017 at 3PM EDST. Please e-mail all responses to christopher.gundy@va.gov. STATEMENT OF WORK (SOW) 1. SCOPE: The successful offeror shall provide all vehicles, gasoline, insurance, personnel, and directional materials necessary to accomplish irregular courier services for and between the Veterans Resource and Referral Center (VCRRC) located at 301 Piquette, Detroit, MI 48202 and the John D. Dingell VA Medical Center (JDDVAMC) located at 4646 John R., Detroit, MI 48201 Samples may contain infectious agents. 2. SCHEDULE/LOCATION/ITEMS: Work Hours Pick-up and delivery scheduled Monday through Friday, with pick up time at 4pm, VA delivery time, being no later than 5:30pm. The ten holidays observed by the Federal Government are New Year's Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Also, any other day declared by the President of the United States to be a national holiday. 3. INSURANCE The Contractor shall have the sufficient insurance on contractor owned vehicles and personnel. This information shall be included in the quotation proposal for transportation services: written documentation to verify proof of insurance, bonding documents, competency reports, past performance (two (2) years or four (4) most recent contracts) with contact information for verification by contracting officer, background checks, etc. It should be noted that blood substances will be one of the items transported by the contractor. Successful offeror shall meet all requirements of Federal, State or City codes regarding the transportation of hazardous materials and operations of this type of service. Associated staff will maintain current training and certification in accordance with 49 CFR, Parts 171-173. 4. QUALITY ASSURANCE If the contractor fails to furnish the required services within the time frames established, the Department of Veterans Affairs reserves the right to obtain the service from another source and to charge the contractor with any excess cost which may result. Contact The John D. Dingell VAMC VCRRC Chief or designee shall be the main point of contact. Contact information will be provided upon award. Description of Items: All items that contain Veteran identification will be transported in a locked container. The Medical Center will provide the locked container. Items that may be transported include, but are not limited to: Blood and other body fluid samples - to be transported in a locked container specifically designed for blood transportation. Samples may contain infectious agents. The number of items being transported will be counted prior to being placed into the transport container. The transport container will be locked and the number of items and container number will be recorded on a shipping log. The log will be signed by the shipper and courier. When the container is delivered, the container number and the number of specimens received will be recorded, and a receipt signed by the receiver and the courier. In accordance with VAAR 801.603.71 (a) (1) & (2), only designated personnel can receive and inspect packages and deliveries. These logs will be submitted to the COR on a monthly basis. 5. QUALIFICATIONS: Offers will be considered only from those offerors who are regularly established in the business called for and who are financially responsible and have the necessary equipment and personnel to furnish services in the volume required for all the items under this contract. Successful offeror shall meet all requirements of Federal, State or City codes regarding operations of this type of service. Each offeror shall submit, with the offer, a letter in fully describing the location and telephone numbers of his/her establishment where calls are received and vehicles are immediately available for dispatch. The right is reserved to thoroughly inspect and investigate the establishment, facilities, business reputation and other qualifications of any bidder and to reject any bid, irrespective of price, that shall be administratively determined lacking in any of the essentials necessary to assure acceptable standards of performance. Offeror is required to have or complete a Business Associate Agreement (BAA) with the VA. 6. DRIVERS/ATTENDANTS: All drivers shall have attained the legal age (the age of 21 years) and shall maintain neat, clean and professional appearance. A record of each employee as to character and physical capabilities of performing the duties of a courier shall be maintained and made available for inspection upon request. Drivers shall be licensed in accordance with the State of Michigan, to include any special licensure required for the transporting of infectious materials. Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract. Contracted employees will have Special Agreement Check (SAC) background checks and finger printing completed by the Contractor. They will be issued VA Contractor ID badges that they will be required to wear when in VA facilities. Successful offeror shall meet all requirements of Federal, State and/or City codes regarding the transportation of hazardous materials and operations of this type of service. Associated staff shall maintain and provide proof of current training and certification in accordance to the Department of Transportation (DOT) along with 49 CFR, Parts 171-173. This includes the requirement that all drivers shall complete annual required VA training and submit proof of completion of training to the COR. 7. VEHICLES: Vehicles used in the performance of this contract shall be licensed and meet all minimum vehicle requirements as mandated by the State of Michigan, to include a Hazardous Materials/Waste Certificate if required. No Government equipment shall be provided. 8. LIMITATIONS: Under no circumstances shall any person other than the driver ride in a vehicle or handle items being transported. 9. SECURITY: While on VA grounds, contractor shall secure vehicles when left unattended. If vehicle must be left unattended with motor running, a second set of keys will be necessary, so that vehicle can be securely locked.
