- The purpose of Amendment A00002 is to provide responses to contractor questions and to change the quote response date to January 20, 2017 at 12:00 EST. January 17, 2017 Questions: 1. Please Confirm who is responsible for unhooking, removing, reinstalling, and re hook up the computers, office equipment etc? Response: The VAMC IT Department is responsible for this process, this will be coordinated with the VAMC Audiology Department. 2. Who will be responsible for removing all personnel items from desks and cabinets, etc? Response: The employees are responsible for moving any items off the desk or storing it in the desk while the work is being performed. 3. Is the subcontractor to supple any boxes to the government for moving? Response: No, the subcontractor does not have to supply boxes. 4. The Performance Work Station #4 specific tasks, as for Seam Sealer to be installed. This is typically used when installed broadloom carpeting. Please confirm Seam Sealer is to be used on Carpet Tiles? Response: No, the contractor shall utilize the materials required to properly install the carpet tiles. 5. Will the space be occupied during construction? Response: The space will be occupied during hours of operation; therefore, the contractor shall work after hours and weekends. 6. If the new cove base does not cover up the wall where the existing base is, does the contractor own patching and painting above the new 6" Cove base? Response: No 7. Please confirm what operating hours are? Response: 0700 hours 1700 hours M-F and occasionally Saturdays 8. Are there any known Hazardous materials (lead paint, asbestos, etc)? Response: It is unknown if there are any hazardous materials. 9. For Fair pricing, Please Confirm how much floor leveler should be carried for this project? Response: The contractor shall determine the amount of floor leveler required for this project. 10. Will the VA supply the carpet tile adhesive? Response: No 11. Where will the carpet be, on or off site? If it is off site, will we have to retrieve it? Response: On-site 12. Will the VA provide a disposal dumpster? Response: Yes 13. Is this job Davis-Bacon rate? If so can you provide the rate sheet. Response: No, this is a Service contract. Please see attached the Wage Determination WD 05-2255 (Rev.-19).
