This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA248-17-Q-0286 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-94, 2005-95/01-19-2017. Interested contractors may contact Christopher Brailer at 352-381-5730; emailed RFIs are preferred. Responses should be sent to christopher.brailer@va.gov. To receive a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. NAICS and Set-aside: The NAICS is 811219 and the small business size standard is $20.5 million. This procurement is set aside for Veteran Owned Small Businesses. To be considered as a Veteran Owned Small Business (VOSB), the quoter must be registered in the VA s Vendor Information Pages (VIP) www.vip.vetbiz.gov. Any firm that is not considered a SDVOSB under this NAICS code should not submit a response to this notice. Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation and represents the best value to the government. The Service Contact Act, Wage Determination number 2015-2114, Rev. 2, Date 12/13/2016 (See attached Wage Determination) is applicable to this contract. For further details, access the relevant wage determination at www.wdol.gov. Description of Services: Steris Autoclave Maintenance (See attached Statement of Work) PAYMENT: CONTRACTOR REMITTANCE ADDRESS: Effective January 1, 1999 all payments by the Government to the contractor will be made in accordance with Federal Acquisition Regulation (FAR) Clause 52.232-34, Payment by Electronic Funds Transfer-- Other than Central Contractor Registration (31 U.S.C. 3332). DESIGNATION OF CONTRACT REPRESENTATIVE: A VA representative of the Contracting Officer will be designated to represent the Contracting Officer in furnishing technical guidance and advice regarding the work being performed under this contract. The foregoing is not to be construed as authorization to interpret or furnish advice and information to the Contractor relative to the financial or legal aspects of the contract. Enforcement of these segments is vested in and is the responsibility of the Contracting Officer. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include annual and quarterly price (See attached Price/Cost Schedule); any offered discounts; name, address, and telephone number of the offeror; terms of any express warranty; completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items. Interested contractors must submit a copy of any relevant certifications and licenses along with their quote. Quotes must be received February 17, 2017 by 1500 EST. All quotes must be e-mailed referencing to VA248-17-Q-0286 to christopher.brailer@va.gov; Offers received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition.
