Re: Request for Information for Urology Scopes and Accessories Introduction The General Services Administration (GSA) is considering a new requirement to provide urology scopes and accessories to the VA Hospital located in Ft. Harrison, Montana. The Government does not intend to award a contract on the basis of this Request for Information (RFI). The purpose of this RFI is to gather data and current market place information from industry, including small business firms. The information obtained from this RFI will be used to assist in developing the most suitable acquisition strategy. Furthermore, interested parties will not be reimbursed for any costs related to providing information in response to this RFI. The proposed North American Industry Classification System (NAICS) code for these services is 423450, Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers, with a small business size of 200 people. This RFI is meant to achieve the following goals: 1.) To survey the market place to determine if there are firms available who can supply this equipment to the Veterans Affairs Medical Center located in Montana. 2.) Provide information to support the manner in which this requirement is procured, i.e. Federal Supply Schedule, Open Market, Set Asides, etc. A. General Description of the Services Requested The Veterans Affairs Medical Center located in Ft. Harrison, Montana is seeking the following equipment. At this time the VA is seeking equipment that is compatible with their already existing Olympus equipment. The VA is seeking the following: Visera Elite Video System Center, Elite Xenon Light Source, LIGHT GUIDE 5MM X 3M AUTOCLAVE, High Definition LED LCD Monitor, Remote Cable CV-160 for Service Principle, Visera Elite Keyboard, HDVC-12 12FT HD-SDI Cable, Workstation SI STD Set, SLIM FLEX VID URETERO SCOPE, REV, Luer-Split,CYF/URF IRR, Seal Kit, 6-12Fr 12/Box, HD Ultra-light URO Camera, Instrument tray for flexible equipment, ACMI SEAL KIT (0-7FR), 12/BOX, Urology Endoscopic CO2 Regulation Unit, Single CO2 Cylinder Holder, Kit with TCE400 Cart, Trueview II telescope, autoclavable, 30 degree direction of view, wide angle field of view, 2.7 x 70mm working length, Trueview II, trocar spike, blunt, 2.7 x 70mm working length, Trueview II, sheath, standard flow, 2 stopcock, rotatable, for 2.7 x 70mm telescope, and Bridge for Arthrex Cannula.   The equipment must have the following salient characteristics: Scopes: • Should have a vertical range of travel 25’’ at its lowest point and 42. 3’’ at its highest point. Low table • Scopes and towers must be compatible with existing Olympus scopes and towers without compromising the warranty. • Must have letter of compatibility from Olympus CO2 Insufflation: • CO2 regulation and insufflation units must be compatible with Olympus scopes and instruments in order to be used in our current configuration. • Accessories and scopes may not be a system independent of our current working system • Items must not void the warranty of the existing Olympus scope and tower system Thunderbeat: • All-in-one generator system. Can support All forms of energy used in the OR (Monopolar, bipolar, advanced bipolor, ultrasonic and Thunderbeat). This helps reduce the OR footprint by standardizing equipment. • Generator offers more electrosurgical output options (Monopolar and bipolar) than other generators. Gives surgeons the ability to customize and save settings for optimal procedure outcomes. • Thunderbeat hand piece is the only advanced energy device that offers advanced bipolar and ultrasonic energy simultaneously (in one device). This technology is a benefit by offering reliable vessel sealing and rapid tissue cutting at the same time. • Thunderbeat offers more functionality. With Thunderbeat the surgeon can dissect, transect, cut, seal and coagulate tissue. Often these device functions lead to less instrument exchange and time savings for the surgeon and OR staff. • The Thunderbeat capital generator and disposable hand pieces are on contract and will save the VA significant money each time they are used   B. Responses from Industry Responses and input from interested parties is requested. In responding to this RFI please provide the following information: 1. A capability from your company demonstrating that you can provide these supplies, and provide delivery to Ft. Harrison, Montana. 2. Response to the following questions limiting your total response to up to 4 pages (8.5”x11”, Times New Roman font, size 12). C. Questions for Industry 1.) What is your company name, address, point of contact, telephone number, email address, and size of your organization (ie large business, small business) using NAICS 423450, as the basis of your response. 2.) Are you a small business? If yes, what socio-economic category does your business identify as? (Hubzone, 8A, Women Owned Small Business, Veteran Owned Small Business, etc.). 3.) Is your company a VA Certified Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB)? 4.) Are there other brands that are compatible with Olympus? 5.) Is the equipment description and salient characteristics restrictive to a particular brand? If it is please state which brand and why? D.
