- Special Notice Request for Information Only. The Department of Veterans Affairs, VISN 08, West Palm Beach VA Medical Center (WPBVAMC) is looking for certified, Service Disabled Veteran Owned Small Business (SDVOSB) and Veteran Owned Small Business (VOSB), Small business, and/or Large business sources for the following: Community Based Outpatient Clinic (CBOC) Services: Community healthcare provider to improve access and timely delivery of care and services to veteran beneficiaries in their local communities via direct and indirect modalities outlined below. This is for market research purposes only and is not a request for competitive proposals; however, any firm that believes it can meet these requirements, please provide written notification to the Contracting Officer within fourteen (14) days from the date of the publication of this notice. SDVOSB or VOSB must be certified and verified in the U.S. Department of Veteran Affairs, Office of Small & Disadvantaged Business Utilization, Vendor Information Pages web site located at https://www.vip.vetbiz.gov/. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government. Only responses from authorized representatives of the firm will be considered. Adequate knowledge, skills, and abilities providing staff, space, and equipment to support primary care, mental health, and any associated support services will be considered. Responses are due by 3/22/2017 2:30 PM, Eastern Standard Time (EST). The North America Industry Classification System (NAICS) Code is 621498. Business Size Standard is 20.5 million dollars. The contractor will be responsible for all six (6) CBOCs to provide both primary care and mental health services. Access to specialty care may have to be provided on site or required to be available through referral or telehealth. The Primary Care CBOCs will often need to provide home-based primary care (HBPC) and home telehealth to the population of the WPBVAMC to meet the primary care and mental health needs of Veterans who have difficulty accessing clinic-based care. Primary Care in VA includes both medical and mental health care services, as they are inseparable in proving personalized, proactive, patient-centered health care, Scope: The effort shall be performed at the following six (6) locations: Okeechobee, FL Vero Beach, FL Stuart, FL Fort Pierce, FL Delray Beach, FL Boca Raton, FL Specific Tasks: The services provided by the contractor, include, but are not limited to the following: a. Continuum of care from prevention to diagnosis and treatment, to appropriate referral and follow-up. Contractor s outpatient site of care must have the necessary professional medical staff, diagnostic testing and treatment capability, and referral arrangements needed to ensure continuity of health care. The Contractor shall provide services solely dedicated to veterans regardless of gender or age. Those patients needing specialty or follow-up care shall be referred to VA. b. Responsible for meeting or exceeding VA and TJC (or equivalent) standards. c. VHA HANDBOOK 1101.10 Patient Aligned Care Team outlines complete requirements for the PACT model. Contractor shall provide all services in accordance with the handbook. Performance Monitoring: The contractor's timely performance of requested services will be monitored by the Contracting Officer Representative (COR) and or his or her designees. Services and documentation of care provided under the resultant contract shall be subject to quality management and safety standards as established by VA, consistent with the standards published by TJC or equivalent. The contractor shall develop and maintain Quality Improvement/ Quality Assurance Programs and provision of care equal to or exceeding VA Standards. The results of all Quality Improvement activities performed by the contractor involving VA patients will be shared with VA Quality Management Office. Responsibilities of the contractor: It is the contractor s responsibility to provide all staff, space, and equipment to support CBOC services. Conflicts of Interest: Contractor has reviewed applicable Federal, State or local conflict of interest laws to assure that conflict of interest situations do not inadvertently arise during this contract. Confidentiality: Contractor agrees to abide by all applicable federal laws and regulations pertaining to the privacy of patients and or VA employees, including all Health Insurance Portability and Accountability Act (HIPAA) of 1996/Information Security regulations as they are implemented within the VA, during the period of this contract. The contractor also agrees to abide by the confidentiality requirements of the protocols.
