I am looking for a Small Business preferably an SDVOSB that can meet this Brand Name requirement for Dental Units. Brand Name would apply to ALL CLINS listed in the statement of Work/Need. STATEMENT OF WORK Promark Endodontic System Purchase and Delivery STATEMENT OF WORK PART A GENERAL INFORMATION A.1 INTRODUCTION This contract is for the purchase and delivery of three (3) Endodontic Rotary Systems supplied by Tulsa Dental for Butler VA Healthcare located at 353 North Duffy Road Butler PA 16001. A.2 BACKGROUND The Endodontic Systems will be used for efficient, precise endodontic procedures, from full root canal therapy to pulpectomies. After initial pulpal access has been achieved (with use of orifice shaping burs/files), the endodontic motor and hand piece will be used to instrument the root canals, providing a range of rotary and reciprocation settings that will aid in efficient pulpal debridement and shaping an ideal taper for the canals. As the system allows a variety of file series to be used, the provider would select the appropriate file after determining the working length of the canal using the standard 21mm file with apex locator to scout the canal, and then begin to instrument the canal at this working length, going from the primary to larger files depending upon canal shape and width (which would determine the manner of debridement/instrumentation necessary to achieve an ideal canal taper). After profuse irrigation and cleansing with antimicrobial agents, and use of chelators as needed, the instrumented canals would then either undergo a 2 week medicament period (likely with standard calcium hydroxide fill) or be immediately obturated, depending upon initial diagnosis of pulpal pathology and degree of apical inflammation anticipated. The obturation phase of therapy would then consist of fitting the appropriate GuttaCore Pink cone to the canal that corresponds to that canal s master apical file (this cone would in essence be the master apical cone). With Endodontic system, there is no need for lateral condensation of files, which will save operators considerable time and provide a denser canal fill. This cone will then be heated as recommended by manufacturer and fitted to the canal at working length, solidifying and expanding to provide an excellent seal in the canal, which was precisely instrumented to fit this cone. This will result in excellent endodontic fill and create an ideal environment for eradicating pulpal pathology. The ProMark Endodontic System is recommended for use in the Butler VAMC dental clinic because it enables providers to use an obturation system that will improve the seal of root canal systems and reduce the historically gap-prone fill that is placed during obturation. The ProMark Endodontic System s obturator system, using GuttaCore technology, is unique from other systems in that it does not contain a traditional hard center (often plastic) carrier, or core that is made of a material other than Gutta Percha. GuttaCore is an all Gutta Percha obturator, meaning that it can be easily manipulated like any traditional Gutta Percha fill during root canal retreatments or post space preparations- there is no hard plastic center carrier to interfere with traditional techniques used both in retreatment and post space preparation. In the case of GuttaCore, the carrier is Gutta Percha. No additional or extra instruments will be needed in post space preparationor retreatment of GuttaCore root canals, as would be the case in completing retreatments or post space preparations in root canaled teeth with other obturator systems. The ProMark Endodontic System is recommended because it is ideal for concurrent use with GuttaCore obturators, which should allow for uniquely optimal obturation while not sacrificing the convenience/adaptability of traditional Gutta Percha. A.3 SCOPE OF WORK Three (3) Endodontic Systems will be purchased and delivered to the Butler VA Healthcare located at 353 North Duffy Road Butler PA 16001. Delivery shall include onsite unpacking of equipment and training to staff. STATEMENT OF WORK PART B WORK REQUIREMENTS B.1 The vendor: B1.1 Will provide delivery and training of three (3) Endodontic Root Canal Systems to the VA Butler Healthcare Health Care Center at an anticipated delivery date shall be on or about July 10, 2017 that will be coordinated between the vendor and VA Butler Healthcare based on the VA Butler Healthcare Center construction schedule. The contractor shall adhere to the job site requirements listed below: C.1.1. All personnel to adhere to site safety requirements PPE at a minimum to include hard hats, safety glasses, high-visibility clothing, hard sole shoes. C.1.2. All personnel subject to a 30-minute site safety orientation conducted by General Contractor (GC). C.1.3. Vendor responsible for unloading, handling, unpacking; clean up to dumpster provided by GC C.1.4. Vendor to schedule deliveries through VA who would in turn schedule with GC. At the time of these deliveries, most if not all products will have to go through the loading dock, so a schedule will be arranged for dock use. C.1.5. VA to obtain and maintain current certificates of insurance for each vendor C.1.6. Vendor responsible for protecting product after installation C.1.7. Standard work hours are Monday Friday, 7:00 AM 3:30 PM C.1.8. Contractor shall provide proof of insurance to COR before any work starts STATEMENT OF WORK PART D SUPPORTING INFORMATION D.1.
