Attachment 1: Must submit this completed form with quote RFQ Document: Shall Read RFQ Document ***Shall read RFQ document*** The Federal Bureau of Investigation intends to procure a SEM Solid State Disintegrator for the office located in Los Angeles, CA. Please see below for a list of item specifications. This is a total small business set aside. NAICS Code for this requirement is 333999 with a size standard (in number of employees) of 500. Quotes will be evaluated on lowest price, technically acceptable. All vendors shall be signed up and have an active DUNS number in SAM.GOV to quote. There will be a single award for all items. All quotes shall be firm fixed price. Quoted product shall meet all the required specifications below to be considered for award. Failure to do so will result in the vendor's quote being considered not technically acceptable. Only vendors located in the U.S. will be considered. Purchase Orders shall be accepted for purchases. Delivery shall be performed within 60 days or less. This Request for Quote falls under FAR Part 12 & 13. Please make sure you fill out the Self-Certifying Document (attachment 1). Failure to submit both required documents will result in the vendor being considered nonresponsive to the RFQ.
