- THIS IS A SOURCE SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The VISN 19 Rocky Mountain Network Contracting Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to locate qualified firms to provide the services detailed below. The Government will use responses to this notice to make to appropriate acquisition decision. All interested firms who can meet the requirements stated below should respond, in writing (via email), with information describing their interest, capabilities, geographic location and indicate which socio-economic business category for which they qualify to the Contract Specialist Sura-Quay M. Arbon, Sura-Quay.Arbon@va.gov, no later than 9:00 a.m. (Mountain Time), March 27, 2017. FBO Sources Sought Notice VA259-17-N-0325 LEICA BIOSYSTEMS MAINT AGREEMENT SOURCES SOUGHT has been assigned and should be referenced on all correspondence regarding this announcement. The NAICS Code 811219 is applicable to this acquisition. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract. Location of service: VA Medical Center Salt Lake City 500 Foothill Drive Salt Lake City, Utah 84148 General Requirements: The Contractor shall provide all parts, materials, labor, supervision, test equipment, technical manuals, and transportation necessary to perform two preventive maintenance service visits per year and emergent repair service as required on the Government owned LEICA BOND MAX (Equipment No. 11383668). Mandatory Requirements: a. The Contractor's personnel shall include a certified and experienced Field Service Engineer capable of making any required repairs or performing preventative maintenance on the LEICA BOND MAX Must be an authorized Leica Biosystems distributor, authorized to perform service on this equipment. c. Service provided must meet the Leica Biosystems On-site Service Contract Package: Gold Service Package for the LEICA BOND MAX.
