Attachment G, Price Data Sheet In the event of technical issues on the part of OET that prevent an offeror to successfully entry offer data on OET, the offeror must immediately notify both Government’s Points of Contact via email, to request allowance for offeror’s manual offer submission using Attachment G, Price Data Sheets. Nevertheless the offerors are still responsible for entering offer data thru OET upon its return to the fully functioning state. Attachment F Offeror Submission Package Attachment E PCS OET Vendor Guide. How to use OET. Attachment D Requesting the OET Vendor Role in AMPS. Attachment C How to register in AMPS. Solicitation SPE600-17-R-0210 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement, and its attachments, constitute the only solicitation. Proposals are being requested for fuel requirements for Customer Organized Group (COG) 3, which includes the states of Delaware, Indiana, Ohio, Maryland, Tennessee, West Virginia, Virginia, Kentucky, and District of Columbia. (ii) The Solicitation number is SPE600-17-R-0210, and is issued as a Request for Proposal (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) The NAICS code for this procurement is 324110. This acquisition has been partial set-aside for Service-Disabled Veteran-Owned Small Businesses and the remaining line items are issued under full and open competition. (v) See Attachment A, "Schedule by State" for a list of contract line item numbers, quantities, and units of measure. (vi) Description of Items Being Procured: This acquisition is for distillates and residuals for Department of Defense and Federal Civilian Agencies in the states of Delaware, Indiana, Ohio, Maryland, Tennessee, West Virginia, Virginia, Kentucky, and District of Columbia. (vii) Ordering Period: August 01, 2017 through July 31, 2022. (viii) FAR 52.212-1, Instructions to Offerors - Commercial Items applies to this acquisition with addenda. (ix) Proposals will be evaluated pursuant to FAR 52.212-2, "Evaluation - Commercial Items," and FAR 15.101-2, "Lowest Price Technically Acceptable Source Selection Process." In order to be considered for award, an offeror must meet or exceed technical acceptability standards for the following non-price factor: Supplies; Terms and Conditions. (x) Offerors shall provide a completed copy of all required Representations and Certifications with the offer. All representations and certifications can be found in Attachment F, "Offeror Submission Package COG 3." (xi) FAR 52.212-4, "Contract Terms and Conditions -- Commercial Items" applies to this acquisition with addenda. (xii) FAR 52.212-5, "Contract Terms and Conditions Required to Implement Statutes Or Executive Orders -- Commercial Items," applies to this acquisition.
