- Occupational Health Services Support Combined Synopsis/Solicitation Number: W912HQ-17-T-0015 - Request for Offers This is a combined synopsis/solicitation for commercial items prepared in accordance with the format FAR 12.603 - Streamlined Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Offers are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95, dated January 13, 2017. The requirement is for a 100% small business set-aside under NAICS code 621111, Offices of Physicians. The size standard for this NAICS codes is $11 million. Description: HQUSACE-SO is responsible for providing policy, programs, technical services, and oversight related to safety and occupational health matters in support of USACE missions. HQUSACE-SO is located at 441 G Street NW, Washington DC. As a general overview, the services provided under this contract include acting as an Occupational Health advisor to the USACE Command Surgeon and Certified Industrial Hygienist; making deployment suitability determinations for USACE employees based on information in the Emergency Response Medical Screening Questionnaires; coordinating the medical examination and making deployment suitability determinations for the USACE Urban Search and Rescue Team; reviewing and providing comments on Occupational Health policy and regulations; and responding to a variety of occupational health questions/inquiries as needed.
