Background: On 17 NOV 2016 the USNS Robert E. Peary (T-AKE 5) had a failure of the No. 4 cargo crane while performing the 125% (27,600lb) dynamic weight test in accordance with reference (a). During the weight test the winch hydraulic motor had a catastrophic failure in which it came apart. The failure resulted in the load test weight having an uncontrolled free fall. Military Sealift Command (MSC) N722's Marine Engineering Division is tasked to perform a Root Cause Analysis to ascertain the reason for the failure. The objective of this tasking is to: 1. Complete detailed inspection, testing, and analysis of the components associated with the crane failure. 2. Provide a technical assessment as to the possible root causes of the failure. 3. Provide recommendations on inspections and maintenance to reduce the likelihood of another failure. 4. Review reference (a) to determine if the testing is exceeding the design limits of the crane or potentially causing damage to the crane components. The pieces of the winch hydraulic motor that were able to be recovered, the winch, and motor hydraulic hoses will be provided for inspection, analysis, and testing to assist in determining any possible causes or factors that could have resulted in the failure of the crane. The crane certification history is as follows: 1. The ABS registration as a lifting appliance for the T-AKE 5 was June 2008. 2. The date of the last certification in accordance with reference (a) was June 2012. 2. Scope of Work: 2.1. Determine any causal factors that resulted in the failure of the crane. 2.2. Determine if any indication of material degradation can be traced to the lubricating oil by sampling and analyzing in accordance with reference (b). 2.3. Perform a tear down inspection of the winch in accordance with reference (b) to determine condition of the internal components of the winch. 2.4. Perform visual inspection of provide components, and perform dimensional verification of all parts which appear to have failed, or deformed. 2.5. Provide physical property inspection and analysis of any points of failure. 2.6. The results from 2.1, 2.2, 2.3, 2.4, and 2.5 shall be documented and presented with an Appleton Marine, INC and sub-component Original Equipment Manufacturer (OEM) approved expert analysis and identification of possible causal factors, if any. 3. Specific Tasks: 3.1. The Government will ship all materials to Appleton Marine, INC. 3.2. Appleton Marine, INC shall obtain the services of a certified service representative for the Original Equipment Manufacturer (OEM) of the Rex-Roth winch hydraulic motor for the failure analysis. Appleton Marine shall provide written verification that the Rex-Roth OEM representative is in fact a certified Rex-Roth OEM certified service provider qualified to perform the root cause analysis. An Appleton Marine, INC approved qualified technical representative shall coordinate the performance of the failure analysis of the winch and associated components. 3.4. Perform a visual inspection of the winch lubricating oil for signs of debris. If possible take a sample of the lubricating oil from the winch drum, following the oil sampling procedure on page 4 of publication PB-308 in accordance with reference (b), and analyze it for wear metals content, the correct viscosity, signs of overheating, water and other contaminants. The winch drum has been exposed to the environment. Appleton Marine, INC is to only take samples from areas with the least amount of exposure to environment. 3.5. Perform a tear down inspection in accordance with reference (b) by completely disassembling the winch and inspecting for worn cracked, corroded or distorted parts such as pins, bearings, shafts, gears, brake rotors, brake plates, drum and base. 3.6. Perform a visual inspection of all provided components. Perform a dimensional inspection of any visually failed or deformed pieces found during the inspection. Appleton Marine, INC or the Rex-Roth OEM as applicable shall compare the measurements of the subject equipment dimensions to the manufacturer's details, drawings or manufacturer's published data, including, but not limited to reference (b). Appleton Marine, INC or the Rex-Roth OEM shall provide a written report of findings with an Appleton Marine, INC or the Rex-Roth OEM approved expert analysis of the implications of the findings. The report shall present findings in S.I. units with measurements taken to the decimal place present in the manufacturers detail drawings or manufacturer's published data. 3.7. Provide physical property inspection and analysis of any points of failure including, but not limited to, the motor housing. Appleton Marine, INC or the Rex-Roth OEM shall compare the physical testing of the equipment to the manufacturer's published data material requirements. The analysis shall examine strength and other characteristics that define the material in the material specifications. 3.8. In the final report Appleton Marine, INC shall provide as supported by the information presented in the report the following: 3.8.1. An Appleton Marine, INC approved expert analysis as to the cause(s) of the failure. 3.8.2. Provide any visual comparison between the failed or deformed pieces of the winch and motor assembly and an equivalent piece without failure or deformation. 4. Deliverables: 4.1. Appleton Marine, INC shall submit the deliverables in both hard copy format and electronic format. Appleton Marine, INC shall provide two (2) full size color hard copies of the final report. Appleton Marine, INC shall provide electronic copies of all final deliverables on CD (s). All CD(s) shall be labeled (as a minimum) with the date of each delivery order and list of documents on CD (including drawing numbers if applicable). 4.2. Appleton Marine, INC shall provide a test plan. The test plan shall include, at a minimum, a description of the test or inspection, applicable test or inspection standard, location of test, and name of testing activity, and applicable certifications / qualifications of the testing facility. The test plan shall also include the name(s) of the personnel actually performing or supervising the test or inspection and their respective pertinent certifications and qualifications. 4.3. If applicable, all report deliverable drawings produced shall be prepared and submitted in accordance with, MSC's General Technical Requirement (GTR) No. 5 of reference (d). This GTR can be found at the following website: http://www.msc.navy.mil/N7/gtr/gtr.htm 4.4. The products of this tasking are to be considered unclassified defense technical data and are subject to security restrictions of the contract. Subject to the requirements of the contract, all products produced for this tasking shall be marked as follows: Distribution Statement D. Distribution authorized to the Department of Defense and U.S. DoD contractors only. "WARNING: THIS DOCUMENT CONTAINS TECHNICAL DATA WHOSE EXPORT IS RESTRICTED BY THE ARMS EXPORT CONTROL ACT (TITLE 22, U.S.C. SEC. 2751 ET SEQ.) OR EXECUTIVE ORDER 12470. VIOLATIONS OF THESE EXPORT LAWS ARE SUBJECT TO SEVERE CRIMINAL PENALTIES." 4.5. Upon completion of analyses and submission of deliverables, all parts shall be returned to the Government from disposal. MSC will arrange delivery of the subject pieces and parts to the location designed by Appleton Marine, INC. Once received, the facility that received the delivery shall provide all labor, materials, equipment, special tooling, rigging, staging, shop facilities, crane services, and additional transportation services, as necessary to accomplish all requirements of this statement of work. 5.2. Any information provided or regarding this investigation shall be kept in confidence and is not for public distribution. Any discussions with third parties shall be authorized by MSC TPOC or persons authorized by MSC N72 or the MSC Contracting Officer. 5.3. Appleton Marine, INC shall allow MSC representatives, access to Appleton Marine, INC's facilities, including sub-contractor facilities, for the purpose of witnessing tests and inspections of the subject parts and materials. 5.4. Appleton Marine, INC and sub-contractors shall maintain physical custody of the test subject(s) at all times. The final disposition of the test subject(s) shall be returned to MSC. 5.5. Reference (b) is the applicable technical manual for the T-AKE 5. Reference (c) is applicable to T-AKE 10 and follow on ships. Appleton Marine, INC shall use the appropriate technical manual based on the equipment installed on No. 4 Crane on T-AKE 5.
