Description

STATEMENT OF WORK Hosting of Automated Phone System and Participant Website for the Engaging Veterans and Family Supporters in PACT to Improve Diabetes Management Project Summary The contractor shall work closely with the VA researchers to provide application hosting, production and management, and project maintenance of the automated phone system (Interactive voice response, or IVR) and the password-protected website for study participants. Functionalities includes de-identified, automated emails to non-veteran study participants, faxes sent from the IVR system to VA clinicians, and secure compliant file transfer systems for each environment. The contract period is August 1, 2017 through July 31, 2019. Patient data entered into the IVR system shall be patient first and last name, phone number, gender, smoking status, and birth year. Specific Tasks: The contractor shall work closely with the VA researchers to provide application hosting on Fed-RAMP compliant resources, production and management of the IVR system, and on-going project maintenance. Specific tasks for Engaging Veterans and Family Supporters in PACT to Improve Diabetes Management are, as follows: Hosting, production and management of the IVR system and participant website. The contractor shall arrange for system application support, hosting on Fed-RAMP compliant resources, telephone resources (including local and 800#s), and bill charges quarterly. (August 1, 2017- July 31, 2019) Project maintenance: participant website and IVR system. The contractor shall be responsible for coordinating system updates as necessary, including updates to the participant website following updates to the browsers it is optimized to. The contractor shall bill costs quarterly. (August 1, 2017- July 31, 2019) Period of Performance The contract period of performance shall be August 1 2017 to July 31 2019. Place of Performance The contractor/subcontractor shall conduct all hosting required to meet the current project specifications using their own Information Technology (IT) system located at their physical address, under their exclusive administrative control. The contractor shall not access the VA computer network and shall not be on-site at the VA.
