- Attachment 3 - Representations Attachment 2 - Terms and Conditions Attachment 1 - Statement of Work and Equipment List Request for Quote - SF18 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Request for Quote (RFQ) and the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The purpose of this RFQ is to procure calibration services, routine inspections, and preventative maintenance of balances, weight sets, pipettes, and data loggers on behalf of the Department of Homeland Security (DHS) Transportation Security Laboratory (TSL) in accordance with Statement of Work and Equipment List (Attachment 1). This RFQ will result in a Firm Fixed Price purchase order with one 12-month base year and four 12-month option periods. For invoicing purposes, each 12-month period will be broken into two 6-month units. Offerors are to include a completed copy of the provision at 52.212-3 Offeror Representations and Certifications -- Commercial Items (Jan 2017) with your offer. ( See Attachment 3: Representations ). Offerors are to include a completed copy of HSAR Clause 3052.209-70 Prohibition on Contracts with Corporate Expatriates (Jun 2006) with your offer. ( See Attachment 3: Representations, p. 22 ) Instructions to Prepare Quotations and Evaluation Factors: Instructions: RFQ responses are due 04/21/2017 at 2:00 PM EST. RFQ responses must be submitted by email to: Danielle.Lachter@hq.dhs.gov. The Contractor must demonstrate the ability to calibrate all equipment types in the required ranges (Max 10 pages). The Contractor must provide proof of ISO 17025 accreditation. Copies of all current accreditation shall be submitted with the quote. Offeror must provide a firm-fixed priced quote for all Contract Line Item as provided in the SF 18. Offerors are required to have an active System for Award Management registration, no active exclusions, and BOTH FAR AND HSAR representations and certifications completed by the quote submission due date in order to receive a purchase order award. Ensure that all representations and certifications, required by this Request for Quote, are included with your quote submission. Evaluation: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer is the Lowest Price Technically Acceptable quote. In order for a quote to be considered technically acceptable, the offeror must be able to meet all requirements of the SOW including the ability to calibrate all equipment from the equipment list, as well as provide proof of accreditation as described above. The Government will evaluate offers for award purposes by adding the total price for all contract line items for the total price for the basic requirement. Award will not be made to any offeror who provides other than a firm-fixed priced quote. Award will be made to the offeror that offers the lowest price, so long as they are found technically acceptable. PLEASE SEE THE ATTACHED RFQ FOR ADDITONAL INFORMATION.
