- RFQ VA259-17-Q-0417: Surgical Instruments (able to be disassembled) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is: VA259-17-Q-0417, and it is issued as a request for quotation (RFQ) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular, 2005-95, effective January 13, 2017. This solicitation is 100% set-aside for Service Disabled Veteran Owned Small Business. The one piece traditional design allows for microorganisms on a contaminated object (Bioburden) to become trapped inside the instrument leading to non-sterile instruments. The new mandated take apart Kerrison Ronguers allows thorough decontamination by allowing the Surgical Processing Department to take apart the instruments and clean appropriately mitigating contamination. Kerrison s are Rongeurs used in Ortho Spine Surgery and Neurosurgery; they are used to bite off vertebral bone (or skull in craniotomy) as well as disc and surrounding ligament tissue. Our most common procedures in which they are used include laminectomy, foraminotomy, discectomy, cervical and lumbar fusions and spinal cord stimulator placements. The requirement shall meet the following salient characteristics. Salient Features: Able to be disassembled, made from German stainless steel, Titanium Nitrade Coating, standard foot plate, with ejector in length sizes below with bite sizes in mm as stated below as well. The requirement will be for 6 EACH. Date and place of delivery (FOB Destination): 05/11/2017 VA Health Care System 500 Foothill Drive Salt Lake City, UT 84148-0001 The provision at 52.212-1, Instructions to Offerors Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Lowest Price Technically Acceptable (LPTA) Note: This is an open market acquisition, however FSS pricing is acceptable and encouraged, if available. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable, if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror, within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items, with the offer. The clause at 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition. Any additional contract requirements or terms and conditions: None. The Defense Priorities and Allocations System (DPAS) does not apply.
