Page 1 of 1 ***** Begin Word Document - 'FBO-DESCRIPTION' ***** Page 2 of 2 Department of Veterans Affairs, Network Contracting Office 18 intends to award on a sole source basis [IAW FAR 6.302-1 and 13.106-1(b)] with the following vendor. Medtronic USA Inc. 710 Medtronic Parkway Minneapolis, MN 55432-5603 Medtronic is currently identified as the only responsible source of licensing and support of the Medtronic Auto-log Cell Washers in accordance with the manufacturer s proprietary system statement. Services covered under this service contract are to include the following, but are not limited to: Scheduled maintenance to include an itemized list of the procedures performed, including electrical safety. Services shall include, but need not be limited to, the following: Cleaning of equipment. Reviewing operating system software diagnostics to ensure that the system is operating in accordance with Equipment Performance Calibrating and lubricating the equipment. Performing remedial maintenance of a non-emergent nature Testing and replacing faulty and worn parts and/or parts likely to become faulty, worn, or failed Inspecting and replacing, where indicated, electrical wiring and cables for wear and fraying Inspecting and replacing, where indicated, all mechanical components for mechanical integrity, safety, and performance Returning the equipment to the operating condition as stated in Equipment Performance Section Providing documentation of services performed (2) Quality Assurance Review per year to ensure that equipment performs in accordance with Equipment Performance Section of this document. The Medtronic systems are currently used primarily for offering extracorporeal clinicians the benefits of auto transfusion during cardiopulmonary bypass. The primary function of the system is to process blood collected from a surgical site, to produce washed red blood cells for return to a patient at a later time. This acquisition will be a single purchase of Preventative Maintenance services on 3 Auto log cell washer machines. Period of performance will last one (1) calendar year from date of award. This procurement is being conducted under simplified acquisition procedures (IAW FAR 13.5). There are no set-aside restrictions for this requirement. The intended procurement will be classified under NAICS 811219 with a Small Business Size Standard of $20.5 million. This notice of intent is not a request for competitive proposals and no solicitation document exists for this requirement. However, parties interested in responding to this notice shall submit proof of certification, including costs, sufficient to determine capability in providing the same service. All capability statements received by closing of this notification of this synopsis will be considered by the Government. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government.
