- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is for market research to: make appropriate acquisition decisions and to secure knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses; as well as: 8(a), HubZone and other Small Businesses interested and capable of providing the product(s) and/or services described below. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business; and (i) must provide a capability statement that addresses the organizations qualifications and the ability to provide the product(s) and/or services described below. THIS PROCUREMENT IS FOR NEW EQUIPMENT ONLY; NO REMANUFACTURED OR "GRAY MARKET" ITEMS. ALL EQUIPMENT MUST BE COVERED BY THE MANUFACTURER'S WARRANTY. The New Mexico VA Health Care System (NMVAHCS), located at 1501 San Pedro Dr SE, Albuquerque, NM, 87108-5128, is seeking potential qualified contractors who can provide the: Dexcom G4 Platinum System, with accessories, brand name only. All equipment will be shipped directly to: Phoenix VA Healthcare System, 650 Indian School Rd, Phoenix, AZ, 85012-1892. Any prospective vendor must be authorized/certified to sell the products or services described and show proof of that authorization at the time of responding to this Sources Sought announcement. Important: The Government is not obligated to, nor will it pay for or reimburse, any costs associated with responding to this sources sought announcement request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to the information contained in this announcement, in the event a solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Businesses. However, if responses from Service Disabled Veteran Owned Small Business firms prove inadequate, an alternate set-aside, or, full and open competition, may be considered. No subcontracting opportunities for this procurement are anticipated. The North American Classification System (NAICS) code for this acquisition is 334510, Electromedical and Electrotherapeutic Apparatus Mfg (1,250 employees). Notice to potential vendors: All vendors who provide products or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. Description of what the equipment being requested is: The Dexcom G4 Platinum System is a continuous glucose monitoring device that provides dynamic glucose information on a real time basis. The system includes the receiver, charger, USB cable and one pack of shields. 2. Description of how the equipment being requested operates: The CGM monitor or insulin pump is small, discreet, and easy-to-wear. It can be attached to your belt, hidden in your pocket, or placed under your clothing. This component will show your current glucose levels and your historical glucose trends. It also notifies you before you reach your low or high glucose limits and if your glucose level rises or falls too quickly. The CGM transmitter is a small, lightweight device that attaches to the glucose sensor, gathers your glucose data, and sends it wirelessly to the glucose monitor unit. The Medtronic transmitter is waterproof and can be worn while swimming, bathing, or showering without worries. The glucose sensor is inserted under the skin to check glucose levels in tissue fluid. The glucose sensor has a small adhesive (sticky) patch to hold it in place for a few days and then it must be replaced with a new sensor. The glucose sensor is inserted with a needle, which is removed after the glucose sensor is in place. The most common place to wear a glucose sensor is in the abdomen. The glucose sensor is easily inserted under the skin using an insertion device. A sensor is placed into the insertion device, and with a push of a button the glucose sensor is inserted quickly and easily. The device is attached to a patient s skin to be worn 24 hours/7 days a week. It measures blood glucose levels on a continuous basis. It can alert to highs and lows before they happen. It is lightweight and can transmit results up to 20 ft to a wireless device. PHI, PII and III are not loaded into the device but are entered in the uploading process to the software. The device will be assigned to a patient in the software and can be matched in uploading process. 3. Where will the equipment being requested be used:The system will be provided for the patient in the clinic and given instructions for use. Results will be used in determining treatment strategies for the patients in the initiative. 4. Who will be using the equipment being requested: Endocrine clinic and orthopedic surgery providers will be working with the patients who will be outfitted with the device. 5.
