Description

This Sources Sought/Market Survey Notice is being conducted by the Network Contracting Office (NCO) 10 at 8888 Keystone Crossing, Indianapolis Indiana 46240 to identify other sources that are capable of furnishing regulated biohazardous medical waste collection and removal for the St. Joseph County VA Clinic at 1540 Trinity Place, Mishawaka, Indiana 46545. Vendor must be able to perform services at hundred percent and authorized to handle and dispose of medical biohazardous material. The expected period of performance is a base period not to exceed one (1) year, plus four (4) options of one (1) year in length. If this is a service your business can provide, please respond to this Sources Sought Notice by 3:00 p.m. EDT on June 16, 2017. Responses should include: (1) Business Name and Address; (2) Point of Contact Name, Phone Number, and E-mail Address; (3) DUNS and NAICS code; (4) Business Size: SMALL or LARGE; and (5) Type of Business: SERVICE DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUBZone, WOMAN-OWNED, etc. All businesses eligible to provide this services are encouraged to reply as this information may be used to determine potential set-asides for the above-noted service.
