STATEMENT OF WORK for the Hologic Inc. SCOPE: The Southern Arizona VA Health Care System (SAVHCS) Pathology, Laboratory Medicine Service, Cytology Laboratory requires reagents and supplies for preservation of cytologic specimens for in vitro diagnostic utilizing the Liquid- base cytology technology from the Hologic Company. GENERAL REQUIREMENTS: The vendor must provide the facility with Food and Drug Administration approved reagents, disposables, and any consumable parts necessary for T2000 processor The reagents, consumables and supplies must be the same as those that have been validated on the T2000 processor. The Pap Smear test kit must contain: 500 filters 500 vials 500 brush/spatula/broom collection devices 500 glass slides The vendor must be able to sequester lot numbers of reagent test kits The vendor must take orders and directly distribute to ensure there is not a delay in patient testing.
