- Attachment 3 - Past Performance Questionnaire Attachment 2 - Pricing Worksheet Attachment 1 - Requirements Document HSCG23-17-R-MNQ124 RFP Package The U.S. Coast Guard Acquisition Directorate anticipates awarding a Firm Fixed-Price Requirements contract for one (1) base period and four (4) option periods to procure approximately 15,200 USCG Boat Crew Helmets in accordance with Attachment 1 - Requirements Document. This solicitation is being posted in accordance with FAR 15.203 and evaluated in conjunction with FAR 15.101-1. The contract will be issued to one (1) Offeror capable of successfully supplying the helmets, found to be the best value to the Government in accordance with FAR 15.101-1 Tradeoff Process. The contractor shall perform all functions necessary to provide the commercial item described herein. Each deliverable shall be provided as per the scope of CLINs and in accordance with the Requirements Document (Attachment 1) and will be incorporated into the contract upon award. Manufacturing shall comply with all applicable Federal, State, and local laws, regulations, codes, and directives. Contractors shall use the Pricing Worksheet (Attachment 2) and Past Performance Questionnaire Rating Sheet (Attachment 3) in the development of the response to this solicitation. This solicitation is being issued under NAICS code 339920 (size standard 750 or fewer employees) as a total small business set-aside. The complete solicitation is attached.
