- Technical Evaluation Criteria Statement of Work for Clinical Long Term Care Policy and Training Consultant This is a Small Business Set Aside Request for Quotation prepared in accordance with FAR Part 13. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare & Medicaid Services intends to award, under Simplified Acquisition Procedures (Dollar Threshold for Simplified Acquisitions $150,000.00) a fixed price purchase order for "Clinical Long Term Care Policy and Training Consultant" services. A detail Statement of Work and Technical Evaluation Criteria are attached. Responses shall be no more than 10 pages and shall include the following: 1) Technical Understanding and Approach, 2) Personnel, 3) Management, and 4) Price - fully loaded labor rate and anticipated hours. The award will be made on the basis of lowest price technically acceptable meeting or exceeding the acceptability standards for non-cost factors. The period of performance is approximately September 5, 2017 to September 4, 2018. The North American Industry Classification Code (NAICS) is 621330. The award should be made within 7-10 business days of the RFQ response due date. Your quote should include the following additional information: 1. Company Name, Address 2. Contact Person, name, Telephone Number, Fax Number, E-Mail Address 3. Business Size Classification, i.e. Small Disadvantaged, 8(a), Woman Owned 4. DUNS Number 5. Tax ID NO. Note: You must be registered and the registration current in the SAM or System for Award Management in order to receive a federal government award. Your quote must be received by August 11, 2017 no later than 2:00pm. Questions must be submitted in writing only to: Phillip.Harrell@cms.hhs.gov no later than August 8, 2017 by 12:00pm. QUESTIONS WILL NOT BE ANSWERED VIA TELEPHONE. Quotes must be sent electronically in MS Word Format to: Phillip.Harrell@cms.hhs.gov or sent in hard copy to Phillip Harrell, Centers for Medicare and Medicaid Services, Office of Acquisition and Grants Management, 7500 Security Blvd., Baltimore, MD 21244. All responses/quotes submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated.
