Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is VA244-17-Q-1407 and issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. This solicitation is a 100% Service-Disabled Veteran-Owned Small Business Set-Aside. The applicable NAICS code is 339113, and the small business size standard is 750 employees. The Department of Veterans Affairs, Wilkes-Barre VA Medical Center, Wilkes-Barre, PA 18711, has a Brand Name requirement for Endur ID wristbands, P/N XSS-1-LA-1. The requirement is for delivery of 20 boxes (300 per box) per month for a twelve month period beginning October 1, 2017. Delivery shall be made, FOB Destination to: Department of Veterans Affairs Wilkes-Barre VA Medical Center 1111 East End Blvd. Wilkes-Barre, PA 18711 Please include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items, with your offer. A pdf copy of solicitation VA244-17-Q-1407 is available for download with this posting. Quotes are due no later than 12:00 PM EST on Friday, August 25, 2017, and shall be submitted electronically to jennifer.cutshall2@va.gov. Quotes received after this deadline may not be considered.
