- Description: NOTICE OF INTENT TO SOLICIT SOLE SOURCE The Defense Logistics Agency, Troop Support, Philadelphia, PA Medical Directorate intends to award a Firm Fixed Price, Sole Source contract under the authority of FAR 13.106-1(b) to AMO Sales and Services, Inc. 1700 E. St. Andrew Place Santa Ana, CA, 92705 for the procurement of the following brand name treatment and calibration key cards. •1. Product Number: 0040-7000 Description: 10 TX, Addedvue Pak, ST23 Domestic Quantity - 20 cards •2. Product number: 81030 IFLAPSUS Description: 590106AM 20 PROC Pack Quantity - 8 packs (Packs contain 20 each, 20 x 8= 160 cone procedures) This order for brand name treatment and calibration key cards will be placed on a sole source basis with AMO Sales and Service as the following circumstance in FAR 13.106-1(b) applies: only one source is capable of providing the services required at the level of quality required because the servicers are unique or highly specialized. The treatment and calibration key cards are for the AMO brand name femtosecond ophthalmic laser and excimer ophthalmic laser systems already in place at Irwin Army Community Hospital, Fort Riley, Kansas. The absence of these treatment and calibration key cards will stop any procedure from being performed. Thus, only the exact brand name treatment and calibration key cards will meet the customer's needs. The estimated value of this contract is $73,072. Delivery schedule will be discussed prior to issuing the delivery order. The anticipated award date is September 15, 2017. This notice is not a request for competitive quotes; however, that all responsible sources and interested parties may identify their interest and capability to meet the requirements by submitting technical data for this requirement data within 7 calendar days of this notice posting (no later than August 30, 2017), by 12:00 PM EST, to Sean McColgan, via email at sean.mccolgan@dla.mil, or to Ryan Jeffries, via email at ryan.jeffries@dla.mil, which shall be considered by the agency. Please note telephone calls will not be honored. DLA Troop Support intends to solicit this requirement orally in accordance with FAR 13.106-1(c), thus no written solicitation will be issued.
