Description
Codonics Safe Label System Printers Louis Stokes Cleveland VA Medical Center, Cleveland, Ohio This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Government reserves the right to make no award from this solicitation. This is a Request for Quote (RFQ) and the solicitation number is #VA250-17-Q-0790. The Government anticipates awarding a firm-fixed price contract resulting from this solicitation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circulation (FAC) 2005-95 effective January 13, 2017. The clauses may be accessed in full test at this address: http://www.acquisition.gov/FAR/. The North American Industrial Classification System (NAICS) code for this procurement is 339112, Surgical and Medical Instrument Manufacturing, with a small business size standard of 1,000 Employees. This solicitation is Full and Open Competition. Department of Veterans Affairs, Network Contracting Office 10, is soliciting offers to supply the Louis Stokes Cleveland VA Medical Center with six (6) Codonics Safe Label System Printers for the Ambulatory Surgery Center. Respondents to this solicitation must fully demonstrate their capability by supplying detailed quote information along with any other documents necessary to support the requirements below. The selected Offeror shall comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors -Commercial Items; FAR 52.212-3, Offeror Representations and Certifications-Commercial Items - the selected offeror shall submit a completed copy of the listed representations and certifications or confirm that the annual representations and certifications was completed electronically via the SAM website accessed through http://www.acquisition.gov; FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.232-40; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.204-10, 52.209-6, 52.219-6, 52.219-28, 52.219-30, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.222-37, 52.222-50, 52.223-18, 52.225-1, 52.225-13, and 52.232-34. This combined synopsis/solicitation is for the items listed in the table below: 1. OBJECTIVES This activity proposes the acquisition of six (6) Codonics Safe Label System 500i for the Ambulatory Surgery Center. These printers improve patient safety. Joint Commission has a guideline for drug identification and labeling ISO26825, ASTM 4774. These devices help meet this Joint Commission guidelines. This device improves the safety and accuracy of medication management. The printer uses a barcode technology to read information from a drug container and then prints out a color-coded drug label with expiration date and time. This label is used primarily in the labeling of syringes in operating and procedure rooms. This helps reduce drug errors by ensuring that each syringe has a proper label on it. It reduces the likelihood of syringe swaps, mislabeling and illegible labels due to human handwriting. 2. SPECIFICATIONS Manufacturer/Description/Part/Model Number* Qty SL500i Safe Label System Printer 6 SLS500i Admin Mode SGL Unit Li 6 SLS500i 1 Year Warranty 6 SLS500i Years 2-5 Depot Swap Warranty 6 Kit Mounting Arm 1 The Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system such that the OEM warranty and service are provided and maintained by the OEM. All warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. The vendor shall provide new items ONLY; no remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. 3. DELIVERY Contractor shall deliver all equipment to the Louis Stokes Cleveland VA Medical Center, 10701 East Blvd, Cleveland, Ohio 44106, no later than 90 days after receipt of award. 4. EVALUATION CRITERIA/SUBMISSION OF OFFER Evaluations will be conducted under FAR Part 13 and any subsequent award will be made to the offer that provides the best value to the Government. In addition to price, offers will be evaluated on technically meeting the Governments requirement, delivery schedule, and training schedule. In addition to completing the cost/price schedule, contractors shall provide sufficient data/brochures to allow the Government to complete a technical comparison of the items offered to meet the Government s requirements. Failure to do so could result in an unacceptable offer. A proposed delivery/training schedule shall be provided.
