Description
Sole Source Intent/Sources Sought Notification The Regional Health Contracting Office - Central is issuing this Sole Source Intent/Sources Sought Notification on behalf of Evans Army Community Hospital Fort Carson, CO. Based on the responses to this notice, the requirement may be set-aside for small businesses, procured through full-and-open competition, or result in a sole-source contract. This notice is not a solicitation of offers, nor is it a notice to proceed. Evans Army Community Hospital (EACH) has a requirement for the rental of the single-dose high-energy electro-hydraulic ESWT System. Locations include: - Evans Army Community Hospital, Fort Carson CO Items required for rental service: - Extracorporeal Shock Wave Therapy (ESWT) Ossatron Therapy Device Requested rental service includes: - Premier Shockwave mobile OssaTron service - 2 additional years of rental service - Annual Maintenance Salient characteristics are as follows: - Must be an Electro-Hydraulic Device -Treatment times must be under 7 minutes in the Operating Room -Device must be able to deliver between 14kV-28kV of energy operating at 4 Hz - Focal Volume 7.5mm by 7.3mm by 63.6mm for the total volume of 1,823.2mm -Must be FDA approved -Vendor must be on call and available to our facility -Must provide safe, effective, and significant efficacy for treating a wide array of chronic tendopathies and non-union ailments. If your company has the potential capacity to provide the equipment stated above, please provide the following information: 1) Your company name, address, e-mail address, web site address, telephone number, and size (small versus large business, and any other socio-economic categories) and type of ownership for the organization; 2) Tailored capability statements addressing the particulars of this effort, with appropriate documentation supporting claims of organizational capability, to include proof of authorization to provide maintenance and repair services on above-listed equipment THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for proposals; submission of any information in response to this notice is purely voluntary; the Government assumes no financial responsibility for any costs incurred. Submission Instructions: Interested parties who consider themselves qualified to perform the above listed services are invited to submit a response to this Sources Sought Notice by 12:00 p.m. CST, September 8, 2017.
