VA118A-17-Q-0477_1 Combined Synopsis/Solicitation *This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation VA118A-17-Q-0477 is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94/2005-95 effective January 1, 2017. The solicitation is a small business set-aside under North American Industry Classification System Code 332999, All Other Miscellaneous Fabricated Metal Product Manufacturing, and the associated small business size standard is 750 employees. DESCRIPTION: The Government contemplates the award of a requirements type contract in accordance with Federal Acquisition Regulation (FAR) 16.503 for custom rolling aluminum pulpit ladders. Deliveries of ladders will be scheduled by placing orders. The quantity of ladders will be identified in each order. The minimum quantity the Government will order is 1 ladder; the maximum quantity for one delivery is 99 ladders; and the cumulative total maximum quantity for all orders placed is 100 ladders. Quote Submission: The Offeror s quote shall be submitted electronically by the date and time indicated in the solicitation via electronic mail to Terricia.Lloyd@va.gov. The Offeror s quote shall consist of Past Performance, Price, the Quote and Quote Submission Form Attachment. Hyperlinks or embedded attachments are prohibited. To ensure timely quote submission to the Government, offerors are encouraged to register for the Vendor Portal. Late submissions will not be evaluated. To avoid late quote submission, we recommend transmitting your quote file 24 hours prior to the required due date and time.  Timeliness is determined by the date and time an Offeror s quote is received by the Government and not when an Offeror attempted transmission. Offeror s responses shall be submitted in accordance with the following instructions: Format. The submission shall be clearly indexed and logically assembled. Each submission shall be clearly identified and shall begin at the top of a page. All pages of each volume shall be appropriately numbered and identified by the complete company name, date and solicitation number in the header and/or footer. Submission packet shall not exceed 25 pages. All files will be submitted as either a Microsoft Excel (.XLS) file or an Acrobat (PDF) file. Page size shall be no greater than 8 1/2" x 11" with printing on one side, only. The top, bottom, left and right margins shall be a minimum of one inch (1 ) each. Font size shall be no smaller than 12-point. Arial or Times New Roman fonts are required. Characters shall be set at no less than normal spacing and 100% scale. Tables and illustrations can be landscape and font size not less than 8-point. Line spacing shall be single spaced. Each paragraph shall be separated by at least one blank line. Page numbers, company logos, and headers and footers may be within the page margins ONLY, and are not bound by the 12-point font requirement. Footnotes are prohibited. All proprietary information shall be clearly and properly marked. Pages in violation of these instructions, either by exceeding the margin, font or spacing restrictions for a particular volume, will not be evaluated. The page count will be determined by counting the pages in the order they come up in the print layout view. File Packaging. All of the quote files may be compressed (zipped) into one file entitled quote.zip using WinZip version 6.2 or later version or the quote files may be submitted as individual attachments. Content Requirements. Offerors are responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each quote. Past Performance Factor. Offerors shall submit a list of all contracts (including Federal, State, and local government and private) (prime contracts, task/delivery orders, and/or major subcontracts) in performance at any point during the three (3) years immediately prior to the quote submission date, which are relevant to the efforts required by this solicitation. Areas of relevance include corporate experience in the manufacturing of aluminum-welded rolling platform/pulpit-type ladders within the last three years with the delivered product, quantity, and/or contract values being similar to the scope of this requirement; and aluminum welding experience to include step corner welds and pipe-to-pipe joints. Data concerning the prime contractor shall be provided first, followed by each proposed major subcontractor, in alphabetical order.
