Pecos Road, Building 6 North Las Vegas, NV 89086 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, and Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95, dated 01-19-2017. The associated North American Industrial Classification System (NAICS) code for this procurement is 811219, with a small business size standard of $20.5M. The Department of Veteran Affairs Pacific Islands Healthcare System, Honolulu, HI (VAPIHCS) is intending to negotiate sole source award to Carefusion Solutions, LLC for a period of one year for the preventative maintenance of the Pyxis and Smart Remote equipment at the various locations under the VAPIHCS. The services are for the following: Services Line Item Description Quantity Unit Price Total Price 0001 12-Month preventative maintenance and repair services for the Carefusion Pyxis and Smart Remote equipment in accordance with - Attachment 001 Statement of Work. 1 GP The period of performance is: 1 October 2017 to 30 September 2018. Place of Performance: Various VAPIHCS locations in the Pacific region, with the home office location at: Address: 459 Patterson Road City - State: Honolulu, HI Zip Code: 96819 It is anticipated that a firm-fixed price purchase order shall be awarded on a sole source basis to Carefusion Solutions, LLC (Cage Code: 33033) as a result of this synopsis/solicitation. Sources must be certified and authorized by Carefusion Solutions, LLC in providing the maintenance and repair services in accordance with the Original Equipment Manufacture s (OEM) specifications. Concerns having the capability to provide these services, and are authorized and certified by Carefusion Solutions, LLC, are invited to submit offers in accordance with the requirements stipulated in this solicitation. Award shall be made to the lowest price technically acceptable offeror meeting or exceeding the requirements of this solicitation.
