Description

This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number VA262-17-Q-1908 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation. It is the offeror s responsibility to monitor this site for the release of amendments (if any). This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95, effective January 19, 2017. This is a competitive solicitation; 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB). Offeror must be verified SDVOSB at the time of offer submission due date. The North American Industry Classification System (NAICS) code for this acquisition is 541620 Environmental Consulting Services and the small business size standard is $15 million. Offeror must be registered in the System Award Management (SAM) at https://www.sam.gov, in order to be considered for award. See Section B Schedule of Services and Price in Attachment 1 for the contract line item number (CLIN), quantity and unit of measures. Description of services: The Government is seeking for a qualified and licensed contractor to provide a comprehensive asbestos survey services at VA Long Beach Healthcare System. See Statement of Work (SOW) in Section B.3 of Attachment 1. Period of Performance: The work to be performed is anticipated to begin September 25, 2017 thru September 24, 2018. Place of Performance: VA Long Beach Healthcare System, 5901 East 7th Street, Long Beach, CA 90822 FOB Terms: Destination The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instruction to Offerors - Commercial Items (Oct 2015), applies to this acquisition. See addendum to FAR 52.212-1 in Attachment I Section E (E.1) Solicitation Provisions. A similar provision at FAR 52.212-2, Evaluation Commercial Items is used. See Basis for Award in Section E.2 of Attachment 1. Offeror shall include a completed copy of FAR 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer. The clause at FAR 52.212 4, Contract Terms and Conditions --Commercial Items, applies to this acquisition and full text may be accessed electronically at http://farsite.hill.af.mil. See addendum to FAR 52.212-4 in Attachment 1 Section C (C.1). The clause at FAR 52.212 5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition. See Attachment 1 Section C (C.2). Additional contract requirements necessary for this acquisition is attached. See Attachment 1 - Section B (B.4) Special Contract Requirements. Defense Priorities and Allocations System (DPAS) and assigned ratings are not applicable to this acquisition. Offer is due on September 15, 2017 at 2:00 PM PDT.
