- The Department of Veterans Affairs (DVA) is seeking vendors that may be able to perform preventative maintenance on a GEM 4000 Blood Gas Analyzer (shown below); SN: 07070573, to support a competitive procurement. Accordingly, the DVA highly encourages all interested business (large and small) to respond to this notice. This notice does not constitute a Request for Quote (RFQ)/Invitation for Bid (IFB)/Request for Proposal (RFP) or a promise to issue an RFQ, IFB or RFP in the future. Vendor shall provide all parts and service required to bring the medical equipment back up to OEM specifications. Field technician provided shall be fully trained and qualified by the OEM to service this equipment. All work shall be done in accordance with manufacturer specifications to troubleshoot, repair, and calibrate the existing equipment. All responses to this notice must be in writing and include the Notice # VA247-17-N-1099 in the subject line. Additionally, responses must also include: Name of the firm, point of contact, phone number, email address, DUNS number, CAGE code, identify business size (large business or a small business), and if applicable, a statement regarding small business status (including small business type(s)/certifications(s) such as SDB, 8(a), HUBZone, SDVOSB, WOSB, etc.) and the corresponding NAICS code. Identify whether your firm is interested in competing for this requirement as a prime contractor or not. Identify subcontracting, joint ventures or teaming arrangement that will be pursued, if any. Information in sufficient detail regarding previous experience (indicate whether as a prime contractor or subcontractor) on similar requirements (include size, scope, complexity, timeframe, government or commercial), pertinent certifications, etc., that will facilitate deciding your ability to perform these services at fair market value.
