- Question & Comment Template Past Performance Questionnaire Pricing Schedule Combined Synopsis/RFQ HSHQDC-17-Q-00457 This is a Combined Synopsis/Request for Quotation (RFQ) to acquire Mindfulness Training Services for the DHS/OHA in accodance with the attached Performance Work Statement. This is a combined synopsis/RFQ for commercial items prepared in accordance with the format in Suppart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solication; quotations are being requested and a writeen solicitation wll not be issued. The Government intends to award a FFP Purchase Order. The solicitation, HSHQDC-17-Q-00457, is being issued as a Request for Quotaion (RFQ). Provisions and clauses in effect through FAC 2005-95 are incorporated into this RFQ. This solicitation is unrestricted. The associated NAICS code is 611430, Professional and Management Development Training. CLINS, Items, Quantities, and Unifs of Measure are listed in Attachment 4 - Pricinig Schedule. The purchase order shall provide for Mindfulnss Training Services in accordance with Attachment 1 - Performance Work Statement (PWS). The period of performance will be 12 months from date of award with one 12-month option period. The place of performance may include DHS facilities including training academies, retreat centers and/or the Contractor's facilities. Response Due Dates: Questions in response to this combined synopsis/solicitation are due at 4:00pm Eastern Time on September 13, 2017. Quotations in response to this combined synopese/soliciation are due at 4:00pm Eastern Time on September 20, 2017.
