Description
*An Amendment was made on 19 September 2017 to extend the Response Date from 19 September 2017 at 1200 EST to 22 September 2017 at 1200 EST. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is F1D4527235AW01. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-62. This requirement is set aside 100% for Women-Owned Small Businesses. The North American Industry Classification System (NAICS) code is 325120 with a 1000 employee size standard. Award will be based on Lowest Price Technically Acceptable (LPTA). Written proposals are required (oral offers will not be accepted). Only one contract will be awarded. Funds are presently not available for this contract. No legal liability on the part of the Government may arise until funds are made available IAW FAR 52.232-18, Availability of Fund (APR 1984): " Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer." All submitted quotes must be valid through 12:00 AM, 30 October 2017. LINE ITEMS: 0001: Delivery of Dry Ice Pellets. Period of Perfomance: 01 October 2017 - 30 September 2018. 1001: Delivery of Dry Ice Pellets. Period of Perfomance: 01 October 2018 - 30 September 2019. 2001: Delivery of Dry Ice Pellets. Period of Perfomance: 01 October 2019 - 30 September 2020. 3001: Delivery of Dry Ice Pellets. Period of Perfomance: 01 October 2020 - 30 September 2021. 4001: Delivery of Dry Ice Pellets. Period of Perfomance: 01 October 2021 - 30 September 2022. The resulting contract will be a firm fixed price. STATEMENT OF WORK DRY ICE SERVICE 1. DESCRIPTION OF SERVICES The Contractor shall provide the 11 Medical Group (Joint Base Andrews and Joint Base Anacostia-Bolling, DC) with dry ice services on a routine and emergency calls. 1.1. Routine Services: •a. 150 lbs. of dry ice pellets shall be delivered to Joint Base Anacostia-Bolling, DC every Monday morning, except for scheduled holidays, family days, or base closures. •b. 300 lbs. of dry ice pellets shall be delivered to Malcolm Grow Medical Clinic, Joint Base Andrews, MD every Monday morning, except for scheduled holidays, family days, or base closures. •1.2. Emergency Services: •a. The contractor shall be set up to support emergency services and provide delivery within 24 hours of receipt and acknowledgement of emergency service request to the appropriate clinic / facility. 2.0. GOVERNMENT AND CONTRACTOR FURNISHED EQUIPMENT The government will provide storage containers for both sites. 3.0. GENERAL IN FORMATION 3.1. Normal Hours of Operation: 0730 - 1630, Monday - Friday, exclusive of government holidays. 3.2. Delivery Locations: Location 1: Clinical Lab 238 Brookley Avenue, Bldg. 1300 Joint Base Anacostia-Bolling, DC Location 2: Malcolm Grow Medical Clinic 1060 West Perimeter, Rm 1H25 (Lab) Joint Base Andrews, MD 3.3. TSgt Eugene Fajardo, 240-612-1501 Contractor shall provide a point of contact and telephone number to receive all request calls placed by the government. 4.0. SECURITY REQUIREMENTS Contractor must be able to be certified by DBIDS for installation access. The contractor is required to have all drivers obtain a contractor badge so that they may access the installation unescorted to make their deliveries in a timely fashion, and so that the laboratory personnel will not have to escort them onto the base. Contractor shall provide 2 forms of identification (which shall include a valid driver license), social security number, birthdate and full name of employees to gain access to the installation. To operate a vehicle on the installation employees must also have current registration card and insurance on vehicles being operated. Contractor shall comply with all applicable base parking and zoning requirements. End of Statment of Work FAR 52.212-3 "Offeror Representations and Certifications - Commercial Items" applies to this acquisition and offeror(s) must include a completed copy with its offer.
