Description
The Gulf Coast Veterans Health Care System (GCVHCS), Network Contracting Office (NCO) 16, Biloxi, MS, is issuing a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is being issued. The solicitation number VA256-17-Q-1523 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 effective January 13, 2017. The NAICS code is 541380 and the size standard is $15.0 million. This requirement is for 100% Service Disabled Veteran Owned Small Business (SDVOSB) set-aside only and will be firm-fixed-price. The closing date for this notice is 2:00 PM CST Sep. 26, 2017. The requirement is for the following: Contractor shall furnish all labor, equipment, supplies, parts, supervision, and management, necessary to provide testing, inspection, maintenance and repair of the potable water system (water well) services for the G. V. (Sonny) Montgomery VA Medical Center, located in Jackson, MS. See Statement of Work for detailed information. Contractor to provide services in accordance with terms and conditions of the contract for the period of October 1, 2017 through September 30, 2018, to include one (1) twelve (12) month option period. To be eligible for award of a contract, vendor must be actively registered in the SAM database (http://www.sam.gov). Vendor is also required to complete representations and certifications found in the provision at 52.212-3, Offeror Representations and Certifications - - Commercial Items. V. (Sonny) Montgomery VA Medical Center requires a Contractor to furnish all labor, equipment, supplies, parts, supervision, and management to provide Testing, Inspection, Maintenance and Repair of the Potable Water System (Water Well) of the Department of Veterans Affairs, G. V. (Sonny) Montgomery VA Medical Center, 1500 E. Woodrow Wilson Dr., Jackson, Mississippi, in accordance with the specifications, terms and conditions of this contract. All work is to be performed in accordance with (IAW) Federal, State and Local laws and ordinances and as required by the State of Mississippi Department of Health. GENERAL: THE SERVICES PROVIDED UNDER THIS CONTRACT SHALL BE IN ACCORDANCE WITH CHAPTER 02 REGULATION GOVERNING THE CERTIFICATION OF MUNICIPAL AND DOMESTIC WATER SYSTEM OPERATORS LOCATED UNDER TITLE 15 MISSISSIPPI DEPARTMENT OF HEALTH, PART III OFFICE OF HEALTH PROTECTION, SUBPART 72 BUREAU OF PUBLIC WATER SUPPLY. The contractor shall provide Certificates/Credentials as addressed in Section 100.03 of said chapter for Class B, minimum qualifications that the Contractor will be personally responsible for. Contractor agrees to carry out all duties and responsibilities as required by Mississippi State Law (Mississippi Code of 1972, Sec 21-27-201 through 21-27-221) during the term of the contract. Contractor agrees to keep the Contracting Officer s Representative (COR) fully informed of all actions required to comply with all applicable provisions of the Federal and Mississippi Safe Drinking Water Acts and all applicable State Department of Health Rules, Regulations, Policies and procedures established under these Laws to include ensuring that all water quality monitoring samples required by the acts are collected and submitted to the state of Mississippi Department of Health or other certified laboratory for analysis. Contractor agrees to personally inspect and properly maintains the treatment faculties, water well, distribution system, storage tank, control systems, and all related appurtenances of this public water supply to ensure that they are functioning properly according to the specified weekly inspection schedules. Contractor shall have experience servicing medical facilities with well systems as the primary public water supply that also provide water to specialized services such as hemodialysis, etc. Contractor shall have a minimum of five (5) years of experience utilizing and/or trouble shooting with DFA Telemetry / SCADA monitoring and control systems and with Severn Trent Microchem 2 chlorine monitors. Services will include, but will not be limited to: Inspections minimum of three (3) times per week, during normal working hours (Monday Friday from 8:00 AM 4:30 PM). Changing out Chlorine and Phosphate supplies as needed. Regulating and monitoring Chlorine and Phosphate residuals. Maintaining Well Log. Performing all required sampling, ensuring the samples are received by the Mississippi State Department of Health (MSDH), and providing the results to the Contracting Officer s Representative (COR). Performing an annual inspection with MSDH/Water Supply and providing the results to the Contracting Officer s Representative (COR). Performing an annual pump test and providing the results to the Contracting Officer s Representative (COR). Performing annual back flow preventer inspections and providing the results to the Contracting Officer s Representative (COR). Contractor to provide and maintain General Liability Insurance in the amount of 2 million dollars during period of contract. The Certified Operator s principle residence must be located within twenty-five (25) miles of the medical center, must be able to respond within thirty (30) minutes and be available twenty-four (24) hours a day. SCHEDULE OF VISITS: The Contractor shall provide a schedule of when services shall be scheduled to the Contracting Officer Representative (COR) within five (5) calendar days prior to the anticipated service date. Services shall be scheduled so as be performed on definite appointed days and in no event, shall the Contractor perform inspections outside regular agreed upon work hours without prior approval from the Contracting Officer Representative (COR). In the event climate or weather conditions make it unsuitable for services, the entire service date will be re-scheduled to a date and time satisfactory to the Contractor and the Contracting Officer Representative (COR). Postponement will not reflect any penalties assessed to the Contractor or to the Government. REPORTS: Upon completion of any service/work, and within ten (10) business days after such service, the Contractor shall provide a written report of the services rendered to the Contracting Officer s Representative (COR) detailing the services. Any unsafe conditions encountered during the service visit/call must be reported immediately to the Contracting Officer s Representative (COR) or designee, advising the Department of Veteran Affairs of the unsafe conditions. IDENTIFICATION: Contractor s employees must have visible on their uniform, their name and company s logo. CERTIFICATION OF CONTRACTORS: Contractor shall be certified (CLASS B Minimum) to perform Servicing, Inspection, and Testing of Water Wells in the State of Mississippi, by the State of Mississippi Department of Health. The Contractor shall furnish a copy of up-to-date and current State certification (showing categories) to the Department of Purchasing and Contracting and to the Contracting Officer s Representative (COR) upon request and prior to the star of the contract. CONFORMITY TO REGULATIONS: The Contractor shall conform to Federal, State and Local regulations governing Testing and Servicing of Water Wells. HOLIDAYS: The following federal holidays are observed: New Year s Day Memorial Day Columbus Day Thanksgiving Day Martin Luther King Jr. s Birthday Independence Day Veterans Day Christmas Day President s Day Labor Day And any other holiday proclaimed by the President as a Federal Holiday. When a holiday falls on a Sunday, the following Monday will be observed as a legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed by U.S. Government agencies.
