- Small Business and Subcontracting Plan Special General Memorandum 2015-03 Pain Management Tests List of 100 most used Tests IHS Business Associate Agreement DHHS Violence Workplace Supplemental Standards of Conduct Residual Standards of Conduct Courtesy and Respect in the Workplace Wage Determinations The Great Plains Area Indian Health Service (IHS) is soliciting proposals from prospective contractor(s) or firm(s) interested in providing reference laboratory testing and pathology services to the following IHS Health Care Facilities: Standing Rock in Ft. Yates, ND, Belcourt, ND, McLaughlin, SD, Cheyenne River in Eagle Butte, SD, Rapid City, SD, Ft. Thompson, SD, Rosebud, SD, Lower Brule, SD, Sisseton, SD, Wagner, SD, Pine Ridge, SD, Kyle, SD, and Wanblee, SD, The GPA-IHS anticipates awarding a performance based non-personal service type, Firm Fixed Price, Indefinite Delivery/Indefinite Quantity (ID/IQ), performance based non-personal service type of contract from this solicitation. The contract is for one (1) base year, with four (4) Option Years period. The government reserves the right to exercise any one of the options. The base year period of performance is anticipated to begin December 1, 2017 through November 30, 2018. The North American Industry Classification System (NAICS) code for this requirement is 621511. The Small Business Administration (SBA) size standard for the NAICS Code 621511 is $32.5 Million. This requirement is a Full and Open (Unrestricted) competitive solicitation and the offer your firm submits will be used by the Indian Health Service to determine, which offer is the "Best Value" to the government. Be advised that the quantities listed within are, "estimates only" and it is not the intent of IHS to be obligated to order the "maximum quantities".
