Description
This is a sources sought notice only and it is not a solicitation announcement. The purpose of this sources sought notice is to gain knowledge of qualified potential contractors and their socio-economic size classification in accordance with NAICS code 532490. Responses will be used by the Government to make appropriate acquisition decisions. Vendors will fill out the last page of this document completely and send it by email to the above email address. After a review of the responses to this sources sought notice, a solicitation announcement may be posted. A response to this sources sought notice is not an adequate reply to the forthcoming solicitation. The VA Iowa City Health Care System located in Iowa City, Iowa is seeking a contractor to provide preventive and emergency maintenance and sensor calibration of Chemdaq ST1-DAQ Ethylene Oxide detection equipment at the Iowa City VA Medical Center in accordance with the attached equipment list (Attachment 1). Contractor shall furnish all labor, transportation, parts, and expertise necessary to To be considered eligible for consideration, vendors must have a field service representative located with the Eastern Iowa area that has been trained by the Original Equipment Manufacturer (OEM) on the specific model of equipment he/she is being asked to work on. Vendors must provide, upon request, documentation of factory certified service/maintenance training on the specific equipment under the terms of this contract. All parts used shall be OEM parts only. Reverse engineered and/or refurbished parts shall not be utilized under the terms of this contract without explicit permission, in writing, from the Contracting Officer. Subcontracting of any ensuing award of this solicitation will not be allowed without written permission of the Contracting Officer. Preventive Maintenance will be performed by the contractor at the intervals that are in accordance with the manufacturer s then current Preventive Maintenance Checklist (PMCL). Contractor will provide the necessary manpower and supervision to properly execute the maintenance and repair of the equipment listed in Attachment 1. A Field Service Report (FSR) must be generated and a copy given to the Iowa City VAMC s Contracting Officer s Technical Representative (COTR) or designee after each episode of scheduled maintenance. Contractor will furnish any required PM kits needed to perform Preventive Maintenance. All repairs will be performed between the hours of 8:00am and 5:00pm, Monday thru Friday, except Federal Holidays. If work must be performed outside of normal business hours ( over-time work), the contractor must obtain prior approval from the COTR. If such work is at the cost of the Iowa City VA Medical Center, a purchase order number must be obtained by the contractor prior to performing any over-time work. Contractor must furnish all tools and materials (e.g. service manuals, diagnostic software, etc.) required to maintain the equipment in top operating condition. A documentation package acceptable for Joint Commission purposes will be maintained by the contractor and made available to the Iowa City VA Medical Center. Required features include, at a minimum: serial number of equipment serviced, any and all work performed on a particular date and preventive maintenance the contractor will perform PM according to manufacturer s specifications. The contractor will have written procedures to be followed and documented evidence that each piece of equipment has been inspected per those procedures. Equipment may be added or deleted from the service contract as needed upon notification by the Contracting Officer (to be documented in writing via contract amendment). Deleted equipment will be credited in the full amount if deleted before any maintenance or repair has been performed on it. At the conclusion of each scheduled maintenance visit, the contractor will provide a written service report indicating the date of service, the model, serial number, and location of equipment serviced; the name of the representative, and the services performed and parts replaced. The contractor will not be held responsible under the terms of this contract for repairs to equipment necessitated by user abuse and/or misuse of equipment. Exclusions: Service does not include electrical work external to the equipment.
