Page 1 of 3 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number: 36C246-18-Q-0046 Solicitation Type: Request for Quotation (RFQ) The solicitation is being conducted under Simplified Acquisition Procedures (SAP) FAR Part 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 05/08/2017. This acquisition is 100% set-aside for Veteran-Owned Small Business Concerns that have been verified by the U.S. Department of Veterans Affairs Center for Veterans Enterprise (CVE) in accordance with 38 CFR § 74 VA Veteran-Owned Small Business Verification Guidelines and appear in the Vendor Information Pages (VIP) database found at http://www.vip.vetbiz.gov/. The North American Industry Classification System (NAICS) Code associated with this procurement is 339113 and the small business size standard is 750 employees. In addition, all offerors must be registered in System For Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The items being requested are Continuous Positive Airway Pressure (CPAP) machines and supplies. All items are being requested as Brand Name or Equal. Please refer to the attached document Salient Characteristics Positive Air Pressure (PAP) Devices and Supplies for complete information on the salient characteristics of the items being requested. The Contract Line Item Numbers (CLINS), items, quantities and units of measure requested are listed below. RICHMON, VA 23249 Please mark all shipments to the attention of Prosthetics Department. The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition. Addenda are attached to the provision. The provision at FAR 52.212-2, Evaluation Commercial Items does not apply to this acquisition. Each line item will be evaluated separately based on price. Each line item will be awarded to the lowest priced responsive and responsible offeror on an All or None basis, meaning that the offeror must be able to supply the total quantity requested of the line item to be considered for award.
