- Exhibit B - Reagent Kits Pricing Table Exhibit A - Past Performance Questionnaire Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, Streamlined Procedures for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotes are being requested and a written solicitation will not be issued. The solicitation number is W81XWH-18-Q-0005. The solicitation is issued as a request for quotation (RFQ). Provisions and clauses in effect through Federal Acquisition Circular 2005-95 are incorporated. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The NAICS code is 334516, with a small business size of 1,000 employees. This is a Full and Open Competition requirement; all qualified vendors are encouraged to submit a quote. USAMRAA on behalf of the United States Center for Environmental Health Research (USACEHR) requests responses from qualified sources capable of providing Brand Name or Equal to ITRAQ Reagents in accordance with the minimum essential characteristics (MECs), and Statement of Need (SON). The resultant Indefinite Delivery, Indefinite Quantity Contract (IDIQ) will be on a Standard Form (SF) 1449 which will include any Contract Line Item Numbers (CLINS). The Government intends to award one Indefinite Delivery, Indefinite Quantity Contract (IDIQ), but may award multiple. Requirements will be procured via the award of delivery orders issued against the basic contract(s). The minimum for each awardee is $250.00. The maximum value of all call orders issued as result of this solicitation is $600,000.00 Period of Performance: Base Period - est: November 15, 2017 - November 14, 2018 Option Period 1 - est: November 15, 2018 - November 14, 2019 Option Period 2 - est: November 15, 2019 - November 14, 2020 Option Period 3 - est: November 15, 2020 - November 14, 2021 Option Period 4 - est: November 15, 2021 - November 14, 2022 Customer Location: 568 Doughten Drive Fort Detrick, MD 21702 INSPECTION: Unless otherwise specified in the order, the offeror is responsible for the performance of all inspection requirements and quality control. Other Administrative Instructions: System for Award Management (SAM). Offerors must be registered in the SAM database to be considered for award. Registration is free and can be completed on-line at https://www.sam.gov/portal/public/SAM/; provide DUNS number; Cage Code and TIN. Offerors are reminded to include a completed copy of 52.212-3 ALT I with RFQ response. All clauses shall be incorporated by reference in the order.
