- This is a REQUEST FOR INFORMATION notice for market research purposes only to determine the availability of qualified small business sources capable of providing the full range of services described below. Responses should be provided via email to kevin.pollard2@va.gov no later than 3:00pm central time on 10/30/2017. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. Please clearly identify any information to be considered sensitive or proprietary. Respondents will not be notified of the results of the market research. SERVICES: Statement of Work for Pain Coaching Program Veterans who live with chronic pain issues often find themselves with the pain as the center of their lives and have difficulty integrating chronic pain treatment effectively into their daily lives. The VA provides many services to assist with the assessment and management of chronic pain issues however often it is difficult for Veterans to fully utilize them to their daily life. We are looking to improve the continuity of care by providing individual, weekly contact with Veterans by trained coaches to assist them in developing the needed skills to integrate available services into their daily activities. By providing a positive and therapeutic working relationship between a pain coach and the Veteran, we believe that the Veteran s quality of life will show significant improvement, dependence on narcotic pain medication will decrease, and overall daily functioning will improve. As this program takes 12 months from the time a Veteran starts the program to when they complete the program and that Veterans do not all start at the same time, we are looking to start the program and accept new referrals from 12/1/17-11/30/18 with a maximum of 246 monthly slots available allowing us to offer this program to approximately 20 Veterans. The Veterans that start the service will have access to no more than 12 months of service from their start date. The agency will bill us monthly for all active participants within those limits which means that treatment services can occur anywhere between 12/1/17-11/30/19. Objectives include: Increasing Veteran s ability to function in daily activities Increasing Veteran s self-reported quality of life Decrease in use of opiates for pain management Improved compliance with primary and specialty care programs Improvements in Veterans being active in their own care management Scope of Work: 12 month program Pain Coaching Program that at minimum consists of Once a week 30 minute Individual telephone pain coaching call Once a week 45 minute group telephone pain coaching call Coaches used must have health coaching/education/healthcare professional backgrounds and been specifically trained with a minimum of 20 hours classroom and 6 months practical supervision working with people with chronic pain issues to include training in Motivational interviewing and cognitive behavior therapy Company will invoice VA monthly for each Veteran with an agreed upon and will pro rate accordingly if the Veteran didn t participate the entire month Veteran s will sign a release of information with the company for progress reports as per their guidelines. The company does not have access to any information on the Veteran through VA systems Progress and quality compliance The Company will provide monthly updates on the Veteran involved which will be scanned into the chart Veterans will complete Pain outcomes questionnaire initially, at 6 months and 12 months which will be scanned into the chart Veterans will be given informed consent and contact information for the EOVAHCS pain program to contact with concerns Random audits consisting of phone contact with a Veteran will occur quarterly to determine compliance The C&A requirements do not apply, and that a Security Accreditation Package is not required: The NAICS code to be utilized for this acquisition will be 621498 All Other Outpatient Care Centers and the small business size standard applicable is 20.5. VOSB and SDVOSB eligibility must be verifiable in http://www.vetbiz.gov/. 2. A brief capability statement demonstrating past performance with relevant contract services in Prime Contractor role. Capability Statement shall not exceed more than 3 pages. 3. Responses to this request will be considered for the purpose of determining how the requirements will be stated and how the procurement will be conducted. Respondents should clearly describe their capabilities to include management experience, equipment and personnel or the ability to obtain them.
