INFORMATION PURPOSES ONLY IF SOLICITED, THIS WILL BE A SERVICE DISABLED VETERAN OWNED SMALL BUSINESS (SDVOSB) SET ASIDE. The purpose of this Source of Sought document is to determine what vendors are available to meet the requirement once a solicitation is ussie..1.1. Under the authority of Public Law 104-262, 38 U.S.C. 8153 and Veterans Affairs Acquisition Regulation (VAAR) Part 873, the Contractor agrees to provide pharmaceutical benefits management products and services to VA Beneficiaries at the patients treated at the Charlottesville Community Based Outpatient Clinic (CBOC) 590 Peter Jefferson Pkwy, 2nd floor, Suite 250 Charlottesville, VA 22911, Fredericksburg North Community Based Outpatient Clinic (CBOC) 130 Executive Center Parkway, Fredericksburg, VA 22401), Fredericksburg South Community Based Outpatient Clinic (CBOC) V 10401 Spotsylvania Ave., Suite 300, Fredericksburg, VA 22408, VA beneficiaries based out of Richmond Veterans Affairs Medical Center (VAMC), Richmond, VA 23249 may be approved for this benefit by the Pharmacy Chief, Richmond VAMC. 1.1.2 Only new prescriptions will be filled unless refills deemed necessary are pre-authorized by the Chief, Pharmacy Service, or designee, Richmond VAMC. 2. BACKGROUND INFORMATION: a. Unless otherwise pre-authorized, prescription fills will be limited to a maximum of a 14-day supply. b. Prescriptions will be filled for legend drugs listed in the Department of Veterans Affairs National Drug Formulary. A copy will be provided upon award to the contractor. c. Prescriptions filled under this contract will achieve the largest discounts available pursuant to contractor's agreements with its network pharmacies. d. Generic drugs will be dispensed whenever possible. A product is not considered a generic equivalent unless the strength, quantity, and dosage are the same as the brand name. The generic name must be displayed on all labels for all medications. e. The following drug products will be dispensed by brand name only: Lanoxin, Dilantin, and Synthroid. Warfarin (Golden State generic only) or brand name Coumadin will be dispensed. f. VA beneficiaries will utilize contractor's network of pharmacies. Contractor must be able to provide a minimum of one licensed pharmacy accepting its plan in Emporia, VA. g. After an authorized pharmacy has filled a prescription, the original order will be electronically transmitted to contractor. Contractor will capture all information required by the VA and provide the Richmond VAMC with such data on a monthly basis. In addition to the information contained on the prescription document, the date of filling, quantity provided, and prescription number will be provided. Contractor shall have at least one pharmacy within (5) miles of the Emporia CBOC. h. The contractor shall bill the Richmond VAMC, Richmond, VA. monthly for services rendered. The Richmond VAMC will randomly audit all billings. Pharmacies accepting payment through the contractor must make records available to the auditor in a timely manner at the location where dispensed. 2. BACKGROUND INFORMATION: a.
