Description
Notice of Intent to Sole Source The Naval Medical Center San Diego (NMCSD) intends to award a sole source purchase order under the authority of FAR 13.106-1(b) due to only one source being available to meet the Government's needs, to Hi-Desert Medical Center, 6601 White Feather Rd, Joshua Tree, CA 92252-6607. The requirement is Laboratory testing services including any and all clinical pathology that cannot be perform by Naval Hospital Twentynine Palms (NHTP) laboratory department in accordance with the statement of work. The clinical laboratory testing service provided with this contract is required to have urgent laboratory tests processed and completed within 60 minutes from the time the specimen leaves NHTP laboratory department. This capability is critical as the NHTP is located in a remote location in the desert of Twentynine Palms and laboratory results for urgent testing is required for emergency patient care treatment. See attached Statement of Work. The NAICS Code for this requirement is 621511; Size; $32.5 Million. Period of Performance: 27 November 2017 - 26 November 2018 This notice of intent is not a request for competitive proposals and no solicitation document exists for the requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to perform the required work. Capability statements shall not exceed 5 (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order. Capability statements are due by 04:00 PM Local Time, 16 November 2017. LABORATORY TESTING SERVICES STATEMENT OF WORK 1. The purpose of this contract is to provide clinical laboratory testing not routinely available at Naval Hospital, Twentynine Palms, CA. 2. Frequently ordered tests as listed but not limited to those on the attachment, and shall be performed using blood, urine, serum, body fluids, or stool samples as appropriate. 3. Miscellaneous chemistry, serology, hematology, microbiology and immunity tests, which are infrequently ordered, will be performed either by the contracting laboratory or its subcontractor as appropriate. 4. Naval Hospital, Twentynine Palms, will deliver all specimens for testing. 5. Surcharges for emergency testing will be in addition to routine test costs. 6. Test results shall include the following information: a) Performing laboratory identification/name b) Patient's name c) Test name and identification number d) Expected normal value e) Contract number f) Delivery order number g) Results shall be sequential in CHCS numeric order 7. The contract laboratory shall meet the following requirements for accreditation: a) Shall be accredited by the College of American Pathologists, a state or regional reference laboratory, or licensed under the clinical laboratory improvement act of 1967. The contractor shall submit proof of accreditation with his offer. b) The contractor shall successfully participate in the College Of American Pathologists Comprehensive Survey Program. c) The contractor shall have a federal identification number issued by the center for disease control. d) All Subcontracted referral facilities utilized by the primary contract laboratory shall also meet the above accreditation requirements. 8. The contractor shall provide Naval Hospital, Twentynine Palms a copy of its catalog including any updates published during contract performance period. 9. The contract laboratory will call all results to Naval Hospital, Twentynine Palms and follow with written report faxed to the laboratory at Naval Hospital Twentynine Palms. 10. Unless specifically provided in this contract, the quality of all services rendered under this contract shall conform to the highest standards of the laboratory profession. All services shall be rendered by or supervised directly by individuals fully qualified and licensed as required by law. 11. None of the services required by this contract shall be subcontracted to or performed by persons other than the contractor or the contractor's employees without the prior written consent of the contracting officer. 12. The contractor does not have exclusive rights to the laboratory samples for tests on contract. It is not a violation of this contract if our facility brings new test on board or desires to send specimens to another Naval Hospital. Laboratory testing services shall include any and all urgent clinical pathology that cannot be performed within the Naval Hospital Twentynine Palms (NHTP) Laboratory Department in accordance with the Statement of Objectives. They are to be processed and completed within 60 minutes from time the specimen leaves NHTP laboratory. Vendor will process all urgent test requirements on an as needed basis. The results will be called into the NHTP Laboratory Department l with a hard copy faxed to 760-830-2159. The total number of tests shall not exceed $29,800.00 within the period of the contract. Tests will be billed at Medicare rates in effect at the time of testing. Billing is to be monthly in arrears.
