- Contractor Performance Report - Short Form VAT Reimbursement Processes BoQ Summary Table BoQ_Lideta BoQ_KG BoQ_GD BoQ_Kuni BoQ_Agumamit BoQ_Gult BoQ_Maksegnit BoQ_ABC BoQ_Gelsha BoQ_Gerbicho BoQ_Haro Health centers Water Quality Assurance Plan Health centers Water Supply Technical Specifications Health Centers Water Supply Drawings Health centers Groundwater Investigation Supplementary Report Health Centers Groundwater Investigation Final Report Solicitation - Water Supply for 11 New Health Centers Activity under USAID/Ethiopia Health Infrastructure Program USAID is working in partnership with the Ethiopia Ministry of Health to improve health infrastructure throughout Ethiopia as part of USAID's Ethiopia Health Infrastructure Program (EHIP). The program aims to improve access to and quality of health services in rural areas of the country. The current phase of EHIP includes construction of 17 health centers, which will be owned and operated by Regional Health Bureaus upon completion of construction and handover by USAID. This particular activity addresses the development and provision of clean, safe, potable water at facilities where there is currently no access to a municipal water source. Improving access to water supply is an integral component for sanitation, proper functioning, and future sustainability of the health centers. The health centers identified herein are either recently completed or currently under construction with estimated completion dates ranging from late 2017 to early 2018. Close coordination of activities with ongoing construction sites and with health centers currently in operation will be imperative for the successful completion of this project. This Solicitation sets forth requirements for proposals for a Contract to acquire the construction services of qualified construction firm/s to provide access to water at 11 Health Centers (HCs): Site 1.1 Haro HC, Site 1.2 Gerbicho HC, Site 2.1 Gelsha HC, Site 2.2 Addis Bete Christian, Site 2.3 Maksegnit HC, Site 2.4 Gult HC, Site 3.1 Agumamit HC, Site 3.2 Kuni HC, Site 3.3 Gulem Dejen HC, Site 4.1 Kachena Gudie HC, and Site 4.2 Lideta HC. Activities shall include but not be limited to the design verification and testing, as well as the provision of material, labor, and equipment to construct groundwater wells at the HCs locations as described in Section C - DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK of the Solicitation.
