- ATTACHMENT 1 CLAUSES SF18 This is a combined synopsis/solicitation for commercial supply prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 70Z034-18-Q-PHBB1000 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-52. The associated North American Industrial Classification System (NAICS) code for this procurement is 423610 with a small business size standard of 500 employees. This requirement is a Small Business set-aside and only qualified offerors may submit a quote. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. All Contractors doing business with the Government must be registered with the System for Award Management (SAM). No award can be made to a company not registered in SAM. A Dun and Bradstreet number may be obtained free of charge by contacting Dun and Bradstreet at http://www.dnb.com/us/duns_update/ or 800.234.3867. The U. S. Coast Guard, Honolulu, HI has a requirement for the supply as listed below. Please email quotation, references, and completed 52.212-3 Offeror Representations and Certifications-Commercial Items form to Base Honolulu (s), Attn: Bryan Karaus at Bryan.C.Karaus@uscg.mil as soon as possible, but no later than 11:59 a.m. HST, 01/10/2018. The contractor shall provide the following supply as listed below: Contractor shall supply an electric forklift in accordance with Attachment 1, Minimum Required Specifications and Attachment 2, Clauses REQUIRED SUBMITTALS WITH QUOTE: (a) Complete Standard Form 18, blocks 8, 11(e), 11(f), 13, 14, 15, and 16; and email your quote to Bryan.C.Karaus@uscg.mil. Specify calendar days to complete all work on line 002. (b) All specs and supporting documentation for any equipment proposed as "an equal" substitute to the specified brand name equipment. This is critical to evaluating equipment for acceptance. QUOTE DEADLINE: Quote must be received by 11:59 P.M., 01/10/2018. RFQ EVALUATION: Responses to this RFQ will be evaluated based on the lowest-priced-technically acceptable quote. Technical acceptance will be based on satisfying the required minimum specifications.
