- 1. This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation that will be issued; quotes are being requested and a written solicitation will not be issued. The Government anticipates a Firm-Fixed Price Purchase Order will be issued from this solicitation. 2. This requirement is being solicited as a Request for Quotation (RFQ). The RFQ number is NIH-RFQ-4794006. 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-96, dated November 6, 2017. 4. This requirement is solicited as a small business set-aside. The associated North American Industrial Classification System (NAICS) code for this procurement is 332913. Its associated Small Business Size Standard is 1250 employees. 5. List of Item(s): The vendor shall provide the following item(s) in the quantities specified below: a. 6" G4000 FDA Flanged Ball Valve ----- 8 each b. 4" G4000 FDA Flanged Ball Valve ----- 8 each c. 3" G4000 FDA Flanged Ball Valve ----- 6 each d. 1/2X1/2 Brass Coupling ----------------- 19 each e. 3/4X2 Brass Nipple ----------------------- 15 each f. 1/2X3 Brass Nipple ------------------------ 10 each g. Shipping Costs 6. Vendors's quote shall include unit prices and total amount prices. Vendor 's quote must specify the delivery time and any applicable shipping costs in their quote. The delivery time may be a consideration in determining the purchase order award. 7. Description of Item Requirements General Requirements: The item supplied shall be a new part and must meet manufacturer's performance standards for new parts. The new part shall not have been used previously, remanufactured, refurbished, or reconditioned. Functional Specifications 6" G4000 FDA Flanged Ball Valve; 4" G4000 FDA Flanged Ball Valve; 3" G4000 FDA Flanged Ball Valve; ½"X ½" Brass Coupling; ¾" X 2" Brass Nipple; and ½" X 3" Brass Nipple 8. Delivery and Acceptance Deliveries shall be made by the contractor after receipt of the purchase order to the address/addressee listed below: Delivery Address/Addressee: National Institutes of Health 9000 Rockville Pike Building 13, Platform C Bethesda, Maryland 20892 Unless otherwise specified, deliveries shall be made to the Delivery Point specified above, Monday through Friday (excluding federal holidays, between the hours of 8:30 a.m. and 4:30 p.m. in accordance of the delivery location time zone (Eastern Time, Central Time, or Pacific Time). Supplies or services scheduled for delivery on a federal holiday shall be made the next business day. 52.252-1 Solicitation Provisions Incorporated by Reference (Feb 1998) This RFQ incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. Also, the full text of a solicitation provision may be accessed electronically at this address: www.acquisition.gov/far 9. Provision 52.212-1 Instructions to Offerors - Commercial Items (Jan 2017) is incorporated by reference. 10. Provision 52.212-2 Evaluation - Commercial Items (October 2014) is incorporated by reference. The provision 52.212-2 incorporated above is completed as follows. Quotes received in response to this RFQ will be evaluated using the lowest-priced-technically-acceptable (LPTA) methodology. To be determined "Technically Acceptable" the offeror must meet or exceed all salient characteristics. Award will be made to the vendor whose proposed equipment meets all salient characteristics and whose total price is the lowest quote. 11. Provision 52.212-3, Offeror Representations and Certifications - Commercial Items (Nov 2017) applies to this acquisition. 12. Clause at 52.212-4, Contract Terms and Conditions-Commercial Items (Jan 2017), applies to this acquisition and no addenda to the clause are included. 13. Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Nov 2017), applies to this acquisition. 14.
