- The attached document provides the applicable regulations of FAR 212-5. The attached document provides the job duties and qualification requirement for the requirement. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number and a statement that the solicitation is issued as an Request for Quote (RFQ): 18-245-SOL-00004 (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular No. 2005-95. (iv) Total Indian Small Business Economic Enterprises Set Aside. (v) Line Item 1: Purchasing Agent Hours: 1,080 hours (vi) The Contractor shall provide Purchasing Agent Services to Gallup Indian Medical Center in Gallup, NM in accordance with the attached Performance Work Statement (PWS). (vii) Period of Performance: January 1, 2018 through June 30, 2018. Place of Performance: Navajo Area Indian Health Service - Navajo Area Office; St. Michaels, Arizona 86511. (viii) A statement that the provision at 52.212-1, Instructions to Offerors-Commercial shall apply. (ix) 52.212-2, Evaluation-Commercial Items: The award will be made to the lowest price technically acceptable offeror. To be considered award, the contractor shall provide evidence of knowledge in the Federal Acquisition Policies and Department of Health and Human Service Acquisition Supplemental Policies. The following evaluation criteria shall be used to evaluate offers: credentials, experience, past performance and price. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. (xi) FAR 52.212-4 Contract Terms and Conditions - Commercial Items (Jan 2017) shall apply. (xii) FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Jan 2017) - See attached document titled "52.212-5 Applicable Required Regulations" (xiii) (a) 52.252-1 Solicitation Provisions Incorporated by Reference (Feb 1998) & 52.252.2 - Clauses Incorporated by Reference (Feb 1998) applies to the RFQ. This solicitation incorporates solicitation provisions and clauses by reference, wht the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The offeror is cautioned that the listed provisions may include blocks that must be completed by the offeror and submitted with its quotation or offer. In lieu of submitting the full text of those provisions, the offeror may identify the provision by paragraph identifier and provide the appropriate information with its quotation or offer. Also, the full text of a solicitation provision may be accessed electronically at this address: www.acquisition.gov/far/. (b) The following HHSAR Clauses are incorporated to this RFQ: (1) 352.215-70 Late Proposals and Revisions (Dec 2015) (2) 352.223-71 Instructions to Offerors - Sustainable Acquisition (Dec 2015) (3) 352.226-1 Indian Preference (Dec 2015) (4) 352.226-2 Indian Preference Program (Dec 2015) (xiv) Not applicable.
