- Experience Questionnaire SO Office Plan Service Contract Wage Rates Clauses and Provisions SF-1449 and specifications for janitorial service This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. DATE, TIME AND PLACE OFFERS ARE DUE: ***Quotes are to be submitted NLT, JANUARY 10, 2018, 2:00 pm, MST*** Email submission of quotes is preferred: email to kdecker@fs.fed.us or mailed/delivered to: USDA Forest Service, Attn: Kim Decker, 1803 W. Hwy. 160, Monte Vista, CO 81144 No faxed quote will be accepted. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number 1282AK18Q0008 is being issued as a request for quote. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. This project is set aside for small business concerns under NAICS code 561720 Utilities/Housekeeping Services, small business size standard is less than $18M three year average. SCOPE OF WORK: The Rio Grande National Forest is seeking quotes for janitorial service for our Supervisor's Office located in Monte Vista, Colorado. The building has approximately 12,800 sq. ft. of office space with mostly carpeted areas and some tile in foyers, bathrooms and storage closets. The purpose of this contract is to provide janitorial services. Services shall be provided in strict compliance with the terms, specifications, conditions, and provisions contained herein. The Contractor shall furnish all equipment, transportation, labor, supervision, and supplies (except as specified herein as Government furnished) necessary to maintain the designated building in a clean, sanitary, and acceptable condition. Performance will be according to the standards indicated herein. The contract will commence on January 1 - December 31, 2018, with two option years. See attached Solicitation 1282AK18Q0008 for detailed specifications and instructions. INVOICE/PAYMENT: Contractor will register and process invoices through www.ipp.gov. The following provisions and clauses apply to this acquisition: REFER TO THE SOLICITATION PACKAGE ATTACHED FOR ADDITIONAL PROVISIONS AND CLAUSES. Provision 52.212-1 Instructions to Offerors-Commercial. Addenda A: Offerors shall submit their quote in the following format and the quantities specified: (1) One copy of the Price Quote to include: • Completed/Signed Offer, SF 1449 complete blocks, 12, 17a, 30(a,b,c) • Completed Schedule of Items, pgs. 4-6 • Completed copy of provision 52.212-3, Offeror Representations and Certifications-Commercial Items (located in document titled Terms & Conditions) See attached solicitation for Evaluation criteria: Provision 52.212-2 Evaluation--Commercial Items.
