Wheelchair Transportation Service (Ambulette Service) for VAACC Columbus, Ohio | Sources sought for SDVOSB and VOSB verified vendors that hold a current OMTB certification in the state of Ohio The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO 10) Cincinnati, Ohio 45220-2637 is seeking qualified SDVOSB and VOSB verified vendors who hold a current Ohio Medical Transportation Board (OMTB) certificate from the state of Ohio to perform wheelchair transportation service (also known as ambulette service) and are interested in submitting an offeror for the following anticipated requirement: Short Title: Wheelchair Transportation Service (Ambulette Service) for VAACC Columbus NAICS-485991: Special Needs Transportation Place of Performance: Department of Veterans Affairs | Chalmers P. Wylie Ambulatory Care Center | 420 North James Road | Columbus, Ohio 43219 Anticipated Period of Performance: 1 April 2018 to 31 March 2023 | Base Year and Four Option Years General Requirements: The contractor shall provide all vehicles, personnel, management, transportation, materials, supplies, and equipment necessary to perform contract services as defined and specified in the PWS for wheelchair transportation services (also known as ambulette service) for VAACC Columbus, Ohio. The contractor shall perform services under this contract in strict adherence to all current laws and regulations applicable to the state of Ohio unless supplemented to exceed those requirements as specified in the PWS. The contractor shall comply with all certification, equipment, staffing, recordkeeping and other rules established by the Ohio Medical Transportation Board (OMTB), formerly the Ohio Ambulance Licensing Board, and appropriate parts of Ohio Administrative Code 4766-3 for Wheelchair Transportation Service (Ambulette Service) to include holding a current OMTB certification to perform services in the state of Ohio. Request all interested SDVOSB and VOSB verified prospective offerors email a signed copy of their current OMTB certification to perform wheelchair transportation (ambulette service) in the State of Ohio in Adobe PDF format for the subject anticipated requirement at VAACC Columbus Ohio to bradley.burhite@va.gov not later than 19 January 2018. Only those prospective offerors who are confirmed as a verified SDVOSB or VOSB under NAICS 485991 and submit a signed copy of the requested OMTB certification in response to this sources sought synopsis will be considered responsive for the purposes of this FBO pre-solicitation notice.
